Liability Waiver: By signing below you are acknowledging that you understand that with any physical activity there are inherent risks and if you or anyone in your family should require medical attention as a result in participating in physical activities at JTDA, or at outside related activities such as recital etc. you agree to hold Jazz Tappin Dance Academy and all related agents free of liability of any such injury sustained through participation and you also agree that you/your medical insurance will cover any such related expenses. I also give permission for use of photo, video etc. for media purposes (advertisements in paper, on social media, website etc)

I also give permission for use of photo, video etc. for media purposes (advertisements in paper, on social media, website etc) All tuition payments will be on autopay and automatically taken out on the first of each month (October thru May) If your autopay does not go thru, by the 15th of the month, a $15 late fee will be added. Costume deposits are due in October and November and costume balance must be paid in December.
