PHOTO RELEASE: I hereby grant Ardaigh Suas Academy of Irish Dance permission to use my child’s photograph/video for promotional purposes and/or social media and website content.

RELEASE OF LIABILITY: I am aware of the physical activity involved in dance classes and accept responsibility for any accidents or injuries that may occur at Ardaigh Suas Academy of Irish Dance. I agree to release and hold harmless Ardaigh Suas Academy and its employees from any cause of action, claims, or demands and will not hold any party liable for any personal injury or personal property damage or loss which may occur on the premises.
