Insurance Release, Photo Release, Minor Consent, and Assumption of Risk Statement

I am aware that during musical theatre lessons, rehearsals, and/or other related activities that I am participating in, under the arrangement for JANDK Enterprises INC., dba Northwest Theatre Lab, certain dangers may occur, including but not limited to, physical injury due to or arising from musical theatre lessons, rehearsals, transmission of illnesses, or other related activities. I am aware that adjustments to schedules and services may be made at the discretion of Northwest Theatre Lab during times of pandemic. In consideration of and as part payment for the right to participate in such musical theatre lessons, rehearsals, or other related activities as provided by Northwest Theatre Lab, and its operator, agents, instructors, and other unnamed assistants, I have and do hereby assume all the above mentioned risk, and will hold all the aforementioned people harmless from any and all liability, action, cause of action, debts, claims, demands of any kind and nature whatsoever which may arise of, or in connections with the musical theatre lessons or related activities provided for by Northwest Theatre Lab. The terms thereof shall serve as a release and assumption of risk for my heir, executors, and administrators, and for all members of my family, including minors accompanying me. My child's photo may be used in Northwest Theatre Lab promotional materials, and I grant Northwest Theatre Lab the right to broadcast, exhibit, market, sell, and otherwise distribute the photos/video recordings, either in whole, or in parts, and either alone, or with other products for commercial, and non-commercial purposes that Northwest Theatre Lab in their sole discretion may determine. I understand no compensation of any kind will be made. I further state that I am of lawful age and legally competent to sign this release, that I understand the terms herein are contractual and not a mere recital, I have signed this document of my own free will. This release will be effective for the entire period I am, or my children are on the premises of Northwest Theatre Lab, or a participant in off-facility, or transitional location. Parents or guardians MUST sign for all persons under nineteen (19) years old.

Sessions, Rates, Fees, Costumes, Tickets, and
Discount Policies at Northwest Theatre Lab
Sessions

There are four seasonal sessions at Northwest Theatre Lab:
Summer Session | June 25 - Aug 31
Fall Session | September 1 - December 31
Winter Session | January 1 - March 3
Spring Session | March 4 - June 16

Enrollment
Annual Enrollment Fee
The annual enrollment season at Northwest Theatre Lab runs from June 2023-May
2024.
There is a one-time annual enrollment fee per family of $25 due at the first enrollment of
each annual season

Rehearsal and Camp Deposits
There is a non-refundable $75 deposit for each show rehearsal and camp enrollment
per person. Deposits are due at the time of enrollment and are required to reserve your
space in the cast or camp.

Tuition Fees | Rehearsal Labs and Camps
Tuition is based upon the number of contact hours NWTL has with performers for each
show or camp. Therefore, each Rehearsal Lab or Camp tuition may vary slightly. Please
go to class registration to see the monthly tuition for each Rehearsal or Camp.

Tuition Rates | Classes
Class tuition is billed monthly and is based on the number of classes a family takes
each week. The tuition schedule is available at the front desk.

Costumes + Fees | Rentals
Rehearsal Labs have a costume rental fee. The 2023-2024 costume rental fee is $60 +tax
per student for each Rehearsal and Production Lab, they are enrolled in. Costume rental
fees will be posted to your account during the first month of rehearsals.

Costume Basics are basic costume pieces that are supplied by the performer's family.
Costume basics include shoes, socks and/or tights, tan/dark caramel Leo with clear
straps (girls), black dance/booty shorts (girls), and black compression bike shorts
(boys). Each show may have a specific need such as a different shoe type or color, but
we try to keep it basic throughout all shows as much as possible so that the costume
basics can be used in multiple shows until they have been out-grown. Most costume
basic pieces are worn under the costumes we provide via rental, to ensure all
performers are completely covered at all times and during all quick changes that may
happen during shows.

Show Tickets
Tickets are available for purchase to attend productions 1-month before the show.
Cast Member families have a 24-hour pre-sale window to ensure they have seats to see
their child perform in the production. Tickets are limited during pre-sale to ensure all
Cast Member Families have the opportunity to reserve and purchase a ticket to see the
show. There are 3-7-performances for every production at Northwest Theatre Lab. Tickets
are limited dependent upon cast size. The number of performances are based upon the location of
the performance.

Tickets will be made available to friends, family, and the general public immediately
following the 24-hour Cast Member pre-sale window.
Ticket prices may vary, but are generally $15-18 per ticket and are non-refundable. Babes
in arms do not require a ticket. Tickets are required for children ages 2 and up.

Discount Policy
There are four types of discounts offered at Northwest Theatre Lab. These discounts
are applied by staff after enrollment. The best discount will be applied to your enrollment
as only ONE discount can be applied to your enrollment when requested and payment has
been received by the assigned deadline. Payments paid in full after the deadline do not
receive a discount.

Discount 1: Rehearsal Labs and Classes Paid in Full
When registering if you would like to take advantage of the Paid in Full discount for
Rehearsal Labs and Classes, please send us an email or text message letting us know
that you are interested.
Cast Member Rehearsal Labs: 15% discount when paid in full
Company Member Rehearsal Summer Lab: 15% discount when paid in full
Company Member JTF Rehearsal Lab: 10% discount when paid in full
Classes: 10% off when paid in full

Discount 2: Sibling Discounts for Rehearsal Labs
When you are enrolling one student at full price each additional sibling enrolled as a
Cast/Company Member receives a sibling discount.
Cast Member Sibling Discount: 20% discount when one sibling is enrolled at full price.
Company Member Sibling Discount: 15% discount when one sibling is enrolled as a
Company Member at full price.

Discount 3: Cast Member Perks
During the session, you are a Cast Member the performer in the cast receives 50% off
their first two classes at Northwest Theatre Lab.

Discount 4: Company Member Perks
During the season you are a Company member the performer in the cast receives
100% off their first three required classes at Northwest Theatre Lab.
During the season you are a Company Member the performer receives 30% off all Cast
Member Rehearsal Labs. Excludes all Company Member Rehearsals.

Withdrawals and Refunds:
Class Withdrawals | To withdraw from a technique class we require a written notice 14-days prior to withdrawal. There are no refunds for monthly class fees. A pro-rated credit will be added to your account for class fees or tuition.

Rehearsal & Production Withdrawals | All withdrawals from a rehearsal and production class must occur prior to the first day of rehearsal. There are NO REFUNDS or CREDITS given for rehearsal and production fees once rehearsals have begun.

Video | Photo Release Form

Everyone enrolled at Northwest Theatre Lab will be photographed and video recorded. We will not be able to accommodate enrolled individuals who prefer not to have their photos taken or to be video recorded.
