The World Health Organization has deemed COVID-19 a world pandemic. SoFA is monitoring recommendations from the CDC, AL Dept. of Health, and local officials and will put recommendations into place. However, attending a class/ at SoFA could increase your risk of infection. In consideration of participating in any SoFA class or camp at First
Baptist Decatur, the undersigned acknowledges and agrees to:
-Accept the risk associated with participating with individuals who may have been exposed to and/or have been diagnosed with COVID-19
-Understand and accept that, if social distancing measures are taken, a distance of six feet cannot be maintained at all times. Instructors and staff will at times be required to come closer than six feet to participants. Participants may choose to come closer than six feet to one another.
-Accept and freely assume all such risks, both known and unknown, associated with COVID-19 and other transmittable diseases or viruses, and assume full responsibility for my participation.
-To waive, release, indemnify, defend and hold harmless FBC Decatur, SoFA, the Director and Instructors of SoFA for any injury or illness, contracting the COVID-19 virus, arising directly or indirectly from participating in a SoFA class, activity, or presence on FBC campus.
-Release SoFA and/or FBC of any liability and acknowledge through signature, to understand the terms and risks of participating in any class of the SoFA and hereby freely and voluntarily sign this agreement.

In the event the participant becomes ill or injured in a SoFA class or on FBC premises, the parent/guardian/primary contact will be immediately contacted. If parent is not present and cannot be reached, permission is granted to take whatever steps necessary to administer first aid and consent is provided to initiate emergency medical treatment, including administration of medication as required.
