You must be over 18 to register for an account.

You must agree to all releases, waivers, and policies before enrolling in any classes.

I HEREBY ASSUME ALL OF THE RISKS OF PARTICIPATING IN DANCE CLASSES OR DANCE EVENTS, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective equipment or property owned, maintained, or controlled by them, or because of their possible liability without fault.

I certify that I am/my child(ren) is/are physically fit, have sufficiently prepared or trained for participation in any dance activities or events, and have not been advised to not participate by a qualified medical professional. I certify that there are no health-related reasons or problems which preclude my/my child(ren)'s participation in this activity or event.

I certify that I am/my child(ren) is/are in good health and that I/they do not pose a health risk to the public.

I acknowledge that this Accident Waiver and Release of Liability will be used by the dance center staff for
dance classes or events in which I/my child(ren) may participate, and that it will govern my/their actions and responsibilities at said dance classes or event. In consideration of my application and permitting me/them to participate in dance class or events, I hereby take action for myself/my child(ren), and assigns as follows:
I HEREBY WAIVE, RELEASE, AND DISCHARGE Afredanz Dance Center and all divisions thereof of any liability and responsibility for injuries, sickness, pandemics, accidents, and natural disasters during participation in and/or instruction of dance camps, private lessons, or any dance classes that I/my child(ren) may participate in.

I WAIVE, RELEASE, AND DISCHARGE from any liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for any death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me/my child(ren) including my traveling to and from the dance center, THE FOLLOWING ENTITIES OR PERSONS: Afredanz Dance Center and their staff, volunteers, vendors, or the activity or event holders, activity, or event sponsors, activity or event
volunteers.

I INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in this waiver, release, and registration form from any liabilities or claims made as a result of participation in dance classes or events, whether caused by the negligence of release or otherwise. The accident waiver and release of liability shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.

I CERTIFY THAT I HAVE READ THIS DOCUMENT, AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF
LIABILITY AND A CONTRACT AND I SIGN IT OF MY OWN FREE WILL.

I acknowledge that participation in dance is potentially dangerous and the is an inherent risk of injury involved.

In allowing my child to participate in Afredanz Dance Center activities, I hereby assume all risks associated with the performing arts. I understand the importance of myself and my child following the instructions and rules set by their instructors, and I agree to release Afredanz Dance Center and it's employees of any and all liability which may arise as a result of child's participation in activities at Afredanz Dance Center.

I understand that Afredanz Dance Center does not assume any responsibility for or obligation to provide financial or other assistance in the event of injury or illness, including but not limited to medical, health, or disability insurance or support.

I authorize Afredanz Dance Center to obtain necessary medical or dental treatment, including first aid, ambulance transport, hospitalization, or such other care necessary for my health and welfare in an emergency. If my insurance does not cover emergency treatment that is deemed necessary and sought for me by Afredanz Dance Center, I agree to be responsible for and pay all costs incurred on my behalf.

I release and discharge Afredanz Dance Center from any claim which may arise on account of any first aid, treatment, or service rendered in connection with my participation in Afredanz Dance Center activities or with the decision by any representative or agent of Afredanz Dance Center to consent to medical or dental treatment on my behalf in an emergency.

I understand that Afredanz Dance Center does not carry or maintain any participant health, medical, dental, or disability insurance coverage. I agree to take responsibility for full payment of any emergency medical or dental costs related to my Afredanz Dance Center participation regardless of whether I have insurance coverage.

I acknowledge and consent to allow Afredanz Dance Center to use:
photos and/or videos of my child/myself in publications (such as flyers/brochures),
advertisements (such as newspapers or online ads), our dance center's website, or on social
media sites (such as the dance center’s Facebook or Instagram account). I understand that in
some cases, my child and/or my name may be used. I further understand no financial
compensation will be for use of these photos/videos.

I understand and agree that it is my sole responsibility to safeguard my personal property while attending or
participating in any classes, rehearsals, workshops, or performances. I hereby release Afredanz Dance Center, its directors, event sponsors, staff, and guest instructors/choreographers from all liability for loss or damage to my personal
property while attending or participating in classes, rehearsals, workshops, or performances. I also agree to abide
by any rules, regulations and policies set forth by Afredanz Dance Center.

The registration fee is due when registering a dancer for classes. This annual fee is recurring at the beginning of every new season of dance classes. The registration fee covers nontuition essentials such as insurance, music licenses, studio communication, administration, and keeping monthly tuition lower. There is a discount for families of more than one dancer enrolled. There also is an option to have the registration fee automatically drafted from your account every new season. Registration fees and payment for the tuition of the season are due at the same time of enrollment. Once the registration fee is paid, it is NON-REFUNDABLE for any reason.

I understand the Registration terms of registration.

Tuition fees are based on hours of class/number of classes taken each week. Tuition will be automatically drafted out of the customer’s account on the 5th day of each month. To stop automatic drafts or reschedule drafts for the month, the customer must call the dance center’s office to make arrangements 3 days before the automatic draft date. Payments that are made after the 5th day are considered late. Late payments will be an additional $10 per dancer. If the tuition is not paid by the last day of the month, the dancer will be dropped from their class. If the dancer reenrolls the following month, the customer is still responsible for keeping the account current by paying off the balance owed and another registration fee.
Once the tuition fee is paid, it is NON-REFUNDABLE for any reason. There will be no discounts or prorating for holiday months, bad weather days, and missed classes. Tuition is flat-rated and is based on 4 weeks within a month, even if there are 5 weeks within a month.
An NSF fee of $30 for any returned check transactions will be added to the balance owed.
