In exchange for participation in dance classes and camps organized by BTC Athletics and/or use of the property, facilities and services of BTC Athletics, I agree for myself and (if applicable) for the members of my family to the following:
1. AGREEMENT TO FOLLOW DIRECTIONS. I agree to observe and obey all posted rules and warnings, and further agree to follow any oral instructions or directions given by BTC Athletics, or the employees, representatives or agents of BTC Athletics.
2. ASSUMPTION OF THE RISKS AND RELEASE. I recognize that there are certain inherent risks associated with the above described activity and I assume full responsibility for personal injury to myself and (if applicable) my family members, and further release and discharge BTC Athletics for injury, loss or damage arising out of my or my family's use of or presence upon the facilities of BTC Athletics, whether caused by the fault of myself, my family, BTC Athletics, or other third parties.
3. INDEMNIFICATION. I agree to indemnify and defend BTC Athletics against all claims, causes of action, damages, judgements, costs or expenses, including attorney fees and other litigation costs, which may in any way arise from my or my family's use or presence upon the facilities of BTC Athletics.
4. FEES. I agree to pay for all damages to the facilities of BTC Athletics caused by any negligent, reckless, or willful actions by me or my family.
5. CONSENT. I give consent to the participation of myself and my family in the activity of dance classes and camps, and agree on behalf of the above minor(s) to all the terms and conditions of this Agreement. By signing this release of liability, I represent that I have legal authority over and custody of enrollees.
6. MEDICAL AUTHORIZATION. In the event of an injury to the above minor during the above described activities, I give my permission to BTC Athletics or the employees, representatives or agents of BTC Athletics to arrange for all necessary medical treatment for which I shall be financially responsible. This temporary authority will begin on July 1st, 2022 and will remain in effect until terminated in writing by the undersigned or when the above described activities are completed. BTC Athletics shall have the following powers:
a. The power to seek appropriate medical treatment or attention on behalf of my child as may be required by the circumstance, including without limitation, that of a licensed medical physician and/or a hospital; and
b. The power to authorize medical treatment or medical procedures in an emergency situation.
7. APPLICABLE LAW. Any legal or equitable claim that may arise from participation in the above shall be resolved under Kansas law.
8. NO DURRESS. I agree and acknowledge that I am under no pressure or duress to sign this agreement and that I have been given a reasonable opportunity to review it before signing. I further agree and acknowledge that BTC Athletics has offered to refund any fees I have paid to use its facilities if I choose not to sign this agreement.
9. ENFORCEABILITY. The invalidity or unenforceability of any provision of this agreement whether standing alone or as applied to a particular occurrence or circumstance, shall not affect the validity or enforceability of any other provision of this Agreement or of any other applications of such provision, as the case may be, and such invalid or unenforceable provision shall be deemed not to be a part of this agreement.
I HAVE READ THIS DOCUMENT AND UNDERSTAND IT. I FURTHER UNDERSTAND THAT BY SIGNING THIS RELEASE, I VOLUNTARILY SURRENDER CERTAIN LEGAL RIGHTS.
Refund Policy
Below is our refund policy for different types of fees/charges. Refunds must be requested in writing and emailed to btcathletics@hotmail.com by the deadlines outlined below. All refunds will be approved at the sole discretion of BTC Athletics. Refunds will be made in the form of studio credit unless otherwise discussed with the owner.
Camps and Workshops
Eligible for full credit if requested at least 7 days in advance. If requested less than 7 days in advance, a full credit can be given if the student's spot is able to be filled. If the spot is unable to be filled, student is eligible for a 50% credit.
Classes
Eligible for up to a full credit if the student is unable to attend classes due to an unforeseen incident. Examples include but are not limited to: injury, illness, or a family emergency.
Foreseen absences are not eligible for credits. Examples include but are not limited to: family vacations, sports games, and school commitments.
Costumes
Eligible for full credit if costume has not been shipped from the manufacturer yet. If the costume has been shipped, BTC will make an effort for the costume to be used by another student. If the costume can be used, the costume is eligible for a 75% credit. If the costume has been shipped and it cannot be used by a different student, then the costume will be delivered to the original student as purchased.
Other Miscellaneous Fees
Any processing fees such as but not limited to registration, recital, competition, and choreography fees are considered non-refundable.

If you purchase BTC's products and services, we collect billing and credit card information. This information is used to complete the purchase transaction.

If you would like to stop receiving marketing or promotional communications via email from BTC, you may opt out of such communications by clicking on the UNSUBSCRIBE button or changing your preferences in your personal portal.

Photo & Media Release
For good and valuable consideration, the receipt of which is hereby acknowledged, I hereby grant BTC Athletics permission to use my likeness in a photograph or video in any and all of its publications, including but not limited to all of BTC Athletics's printed and digital publications. I understand and agree that any photograph or video using my likeness will become property of BTC Athletics and will not be returned.
I acknowledge that since my participation with BTC Athletics is voluntary, I will receive no financial compensation.
I hereby irrevocably authorize BTC Athletics to edit, alter, copy, exhibit, publish or distribute this photo or video for purposes of publicizing BTC Athletics's programs or for any other related, lawful purpose. In addition, I waive the right to inspect or approve the finished product, including written or electronic copy, wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photograph.
I hereby hold harmless and release and forever discharge BTC Athletics from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.
