WAIVER AND RELEASE OF LIABILITY
Assumption and Acknowledgement of All Risk: I understand and agree that by participating in any dance lesson or class, workshop, pop up class, rehearsal or performance, there is a possibility of physical injury. I voluntarily agree, therefore, to assume all risks and responsibility for any such injury or accident, which might occur during any Harlen House of Dance classes, lessons, rehearsals, performances, or activities.
Indemnification: I also indemnify and hold harmless Harlen House of Dance, its owners, agents, volunteers, assistants, employees, and/or guest artists against any and all claims, lawsuits, demands, or causes of action whatsoever from any damage, loss, injury, or death to me, my children, or property which may arise out of or in connection with participation in any classes or activities conducted by Harlen House of Dance.
I acknowledge that I have carefully read this “waiver and release of liability†and fully understand that it is a release of liability. I expressly agree to release and discharge Harlen House of Dance, its owners, agents, volunteers, assistants, employees, and/or guest artists from any and all claims or causes of action and I agree to voluntarily give up or waive any right that I otherwise have to bring a legal action against Harlen House of Dance for personal injury or property damage.
I affirm that I am eighteen (18) years of age or older, have read this document, understand its contents and have full legal capacity to sign this document. If I am signing this waiver for my child/children, I certify that I am the parent or legal guardian and have the right to waive these rights.
