I have read all studio rules and regulations and I am fully aware of all policies and I fully understand and agree to abide by them.
I give Raise the Barre Dance Academy permission to use my child’s photographic image and or video, or myself in official Raise the Barre Dance Academy business including but not limited to website, social media, newsletters, recital, etc. I understand that photographic images or videos may be used for promotional purposes without financial compensation, and in some cases, my child and or my name may be used.
In the event that parents or emergency contact can not be reached, in the case of accident or injury, I give my permission for treatment as deemed necessary by staff or emergency personnel. I also release Raise the Barre Dance Academy, it’s staff, and the facility and anyone associated with it of liability in case of injury, accident, or even death to me, my child/family, and anyone I bring with me while on the premises of Raise the Barre Dance Academy including parking lot. I also release Raise the Barre Dance Academy and it’s staff of liability while traveling to and from events, or participating in any events associated with Raise the Barre Dance Academy.
I have received a member's handbook from the front desk or I have downloaded it from the website at www.raisethebarredanceacademync.com. I understand that if my child has to drop from enrollment I must provide the front desk with a drop form in hand NO LATER than the 25th of the month in order to STOP the billing for the following month. I understand that EVERY student MUST have autopay set up with a card on file. I may pay before the 1st of every month either online or in person to avoid auto draft. Auto draft is on the 1st of EVERY month, and I am responsible for keeping my card up to date. Tuition is due on the 1st and if it isn't paid by the 10th, a late fee of $20 per student will be added to my account balance. We are all inclusive however if I start after September 30 then I will have a costume fee that ranges from $25-$100 per class depending on start date to offset the costume fee included in tuition for recital. Please check in the handbook to see what the fee is if this applies to you. You may OPT OUT of recital by seeing the front desk, however tuition will remain the same and you WILL NOT receive costumes. Our premium classes are at a higher tuition bracket so they do not receive a costume in order to keep the tuition at the same rate. These classes include acro/tumbling/contortionist, aerial hoops/aerial silks, cheernastics, and Level Up. These classes may be featured in the recital or have their own showcase at the studio, except for level up. By signing this waiver I agree to abide by all the rules and regulations, and I understand all the fees and policies.
