Waiver of Liability
We look forward to your participation in dance lessons, programs, and activities provided by or associated with Anita's Dance Center, Inc. Please be aware that certain risks are associated with all athletic, artistic, or recreational activities - including dance. Therefore, in order to reduce our cost to you, we ask that each participant, or the participant's custodial parent or legal guardian, carefully review and sign this waiver of liability. You are not required to sign this agreement, You may, for instance, be able to pay more for lessons in exchange for not signing this agreement. In light of the above, I agree to waive Anita's Dance Center, Inc. from liability, and otherwise discharge or release Anita's Dance Center, Inc., in connection with all claims that I may have arising from the participant's association with Anita's Dance Center, Inc.
I understand that I am not waiving any claims that I may have for reckless or intentional acts.

Medical Emergency
The undersigned gives permission to Anita's Dance Center, Inc., its owners and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I hereby declare any physical/mental problems, restrictions, or condition and/or declare the participant to be in good physical and mental health.

Release of Claims: Name/Image/Likeness/Quotations
I hereby grant Anita's Dance Center, Inc. permission to use, exhibit, or distribute my name, image, likeness, or quotations (material) in any of its publications, in any medium, included, print, web, internet, digital, video, audio, or television. I waive any right to inspect or approve the finished product or the medium or use. I waive any right to royalties or compensation arising from the use of the subject material. I make this release voluntary. I agree to release, discharge, and indemnify Anita's Dance Center, Inc. from any claim that may arise from the creation or use of the subject materials.
