INJURY AND INSURANCE
By signing this release, Waiver & Assumption of Risk, (the "Release") I agree that Premier Dance Academy shall not be responsible for any injury suffered by the student ("student"). The Release extends to Premier Dance Academy, it’s owners, instructors, employees, and locations of classes, rehearsals, and performances (all of which are collectively referred to herein as "the studio").
I understand that dance and tumbling activities as conducted and taught at the studio have inherent risks of injury. These risks include muscle pain and pulls, broken bones, ankle injuries, and other personal injury. I understand that these risks cannot be fully eliminated without jeopardizing the essential qualities of the activity. The student's participation in this activity is purely voluntary, and the student elects to participate, and I join in that election, in spite of the risks.
I further understand that I have been advised of the need for the student to be covered by adequate insurance to cover any injury or damage that may be suffered while participating, and I have obtained such insurance or have agreed to bear the costs of any such injury or damage myself.
PROPERTY LOSS
I agree that the studio is not responsible for any property loss or damage suffered by the student or any guests or parents of the student that results upon entry into or presence in the studio or its surrounding property.
COVID AND ILLNESS
In connection with my participation at Premier Dance Academy “The studioâ€, I understand that there is some risk of exposure to Covid-19 and other illnesses.
SOCIAL MEDIA
I agree that Premier Dance Academy may use photographs and videos of the student for advertising purposes, including print and digitally on their website and other social media sites.
PAYMENT
Tuition at Premier Dance Academy is due by the 15th of each month. I agree to pay tuition on time and understand that failure to do so will result in a $25 late fee each month it is overdue.
I understand a registration fee as specified below must be paid to complete my student(s) enrollment.
Registration Fees
· New student Registration fee is $25
· Returning student Registration fee is $15
· New Family Registration Fee (up to 3 children) $45.00. Additional child $10 per child
· Returning Family Registration Fee (2 children): $30.00. (Up to 3 children): $40.00. Additional child $5 per child.
Registration fees are non-refundable.
First month's tuition is due upon registration approval.
I understand my account will be updated in my Danceworks parent portal.
Premier Dance Academy accepts cash, check, and credit cards. A convenience fee of 3% will be added to your total if you choose to pay with a credit card.
I understand that any returned checks will result in a $25 fee.
I understand that tuition is non-refundable and will not be prorated for absence or weather related cancellations.
Recurring payment is available through your Danceworks account. I understand by selecting “use for recurring billing†in my parent portal, I am authorizing regularly scheduled charges to my debit/credit card. I will be charged my monthly tuition amount (plus the convenience fee listed above) on the 15th of each month, or the next business day.
Any unpaid balances will also be charged at the time of their due date (examples include costumes, competition fees, etc.)
I also understand that if I choose to cancel my recurring payments, I am responsible for making this change in my Danceworks parent portal.
I understand costume deposits ($45 deposit per class due October 4th and remaining balance due December 6th are non-refundable and that failure to pay may lead to removal from performance (s) or additional fees that may be associated with ordering late.
I understand if tuition is not received within a 60 day period, my dancer will be sitting out of class until the balance is paid and may lead to removal from a class or performance.
All payments received will be applied to the oldest charge due on accounts. For example- If tuition is past due, and a payment is received on time for a costume deposit, that payment will be applied to the outstanding tuition balance. This policy applies to all accounts.
I understand June tuition is due along with May tuition.
I understand that if I wish to withdraw from a class, I need to provide written notice to frontdeskstaff@premierdancema.com or I will be responsible for tuition charges that are accumulated.
AGREEMENT
I voluntarily agree to assume all of the above risks and accept sole responsibility for any injury to my student(s) or myself (including, but not limited to, personal injury, disability and death), illness including COVID - 19, damage, loss, claim, liability, or expense, of any kind, that I or my student(s) may experience or incur in connection with my student's attendance at the studio or participation in studio class or programs. I acknowledge that allowing my student(s) to dance without a mask or face covering increases their risk of contracting COVID-19 and other illnesses. I agree to promptly notify the studio in the event that my student(s) is considered to be in close contact with someone who has been infected with COVID-19 or tests positive for COVID-19. I also agree to follow the above isolation and quarantine guidelines. On my behalf, and on behalf of my student(s), I hereby release, covenant not to sue, discharge, and hold harmless the studio, its employees, agents, representatives, locations of classes, rehearsals and performances - including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating there to.
I also understand and agree with the payment terms listed above.
I understand the terms of this Release and have had the opportunity to consider and discuss it with such individuals and advisors as I deem appropriate.
