On behalf of myself and my children, I understand that there are risks related to, deriving out of and inherent to the activities provided through Solely Dance Company, LLC. They include, but are not limited to, illness, injury and death. In recognition of this acknowledged risk, I knowingly and voluntarily waive all right and/ or causes of action of any kind, including any and all claims of negligence arising as a result of such activity from which liability could accrue to Solely Dance Company, LLC, it's officers, agents, employees, instructors, subsidiaries, parent corporations, and all affiliated entities (hereinafter collectively referred to as Solely Dance).
I hereby agree to release Solely Dance and hold Solely Dance harmless of all liability, and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risks arising out of active participation in classes, performances and activities through Solely Dance on behalf of myself, the legal guardian, and the participant/s.
I am aware that this is a release of liability and an acknowledgement of my voluntary and knowing assumption of the associated risks. I have signed this document voluntarily and of my own free will in exchange for the privilege of participation.
I also give Solely Dance permission to use my children's picture in or on any form of advertisement or publication on behalf of Solely Dance or an event affiliated with Solely Dance. (Please notify us in writing or through the portal if pictures cannot be publicly displayed for any reason.
The participants for whom I am registering have my permission to participate in all classes held at Solely Dance which may include performances and activities. I warrant the information is complete and correct and that I do have the authority to act and speak on behalf of the participants I enroll in classes at Solely Dance. I further release Solely Dance of all liabilities associated with my child's attendance at Solely Dance.
All tuition & fees will be paid according to the schedule below. Fees will be charged to the credit card associated with your parent portal via Dance Studio Pro on the 3rd of each month. I understand that a payment of all fees is my responsibility and that a $15.00 late fee may be applied to my account if the card on file is declined and cannot be processed. I further understand that it is my responsibility to maintain a valid CC on file in the parent portal. No changes or updates will be accepted by studio phone or at the studio. You may not deduct for classes missed due to illness, vacation, etc. Exceptions will be made on an individual basis if a student has a medical condition that restricts physical activity. A doctor's note must be submitted before a credit can be given. Credits can be used for future classes at Solely Dance.
I understand that the studio has an attendance policy and that in order for my child to participate in the end of season revue my child must not have missed more than 4 unexcused classes. I further understand that it is my responsibility to provide a doctor's excuse or documentation to support an absence to be excused.
I further acknowledge that Solely Dance has a dress code policy in effect that my child must follow in order to receive instruction.
I have read and understood all of the above. I understand that these policies are created to ensure the absolute best training and most positive and rewarding experience for everyone. I understand that failure to meet the conditions set forth in this contract could result in the dancer’s dismissal.
