I understand that there may be some risk of injury associated with participation in dance/fitness training, and I agree to waive any and all claims of liability, release and hold harmless Artistic Dance Connection, Inc. in the event that such an injury may occur.

In the event of an accident or injury when parent, legal guardian, or emergency contact is not available, I give permission to Artistic Dance Connection, Inc. to procure medical attention.

I accept Artistic Dance Connection, Inc. studio policies.

I release Artistic Dance Connection, Inc. its instructors, independent contractors, and all other associates from liability from harm, theft or injury that may be suffered by me and/or members of my family traveling to or from or during participation in activities produced by Artistic Dance Connection, Inc.

I give permission to Artistic Dance Connection, Inc. to use pictures and or video of myself or my child for advertisement and promotion in print, on social media, our website or other media platforms.

I give permission to Artistic Dance Connection, Inc. to communicate with me or my student via email, cell phone, text, and social media platforms.
