Proper behavior is expected of all students before, during, and after class. Teachers have the discretion to dismiss a disruptive student from the class in the event of improper behavior. Students who repeatedly disrupt classes with improper behavior may be discharged from the studio at the sole discretion of the owner, with or without a warning. KPL promotes a safe and supportive environment. Hitting or pushing is strictly against our code of conduct.

On behalf of myself and my children, I understand that there are risks associated with, arising out of, and inherent to the activities provided through KPL Dance Co. LLC, DBA KPL Dance Co., and Brooklyn Athletic Club. They include but are not limited to, illness and injury. In recognition of this acknowledged risk, I knowingly and voluntarily waive all rights and/ or causes of action of any kind, including any and all claims of negligence arising as a result of such activity from which liability could accrue to KPL Dance Co. LLC DBA KPL Dance Co., it's officers, agents, employees, instructors, and all affiliated entities (hereinafter collectively referred to as "KPL Dance Co.")

I hereby agree to release KPL Dance Co. LLC, KPL Dance Co., and Brooklyn Athletic Club harmless of all liability and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risks arising out of active participation in classes performances and activities through KPL Dance Co. on behalf of myself, the legal guardian, and the participant/s.

I am aware that this is a release of liability and an acknowledgment of my voluntary and knowing assumption of the associated risks. I have signed this document voluntarily and of my own free will in exchange for the privilege of participation.

I also give KPL Dance Co. permission to use my child's picture in or on any form of advertisement or publication on behalf of KPL Dance Co. or a KPL Dance Co.-affiliated event. (Please notify us in writing if pictures cannot be publicly displayed for any reason)

I understand that I must remain within five minutes of the premises for the duration of the class.

The participants for whom I am registering have my permission to participate in all KPL Dance Co. classes, performances, and activities. I warrant the information is complete and correct and that I do have the authority to act and speak on behalf of the participants I enroll in classes at KPL Dance Co. I further release KPL Dance Co. of all liabilities associated with my children's attendance at KPL Dance Co.

I understand that tuition is due by the 1st, for the upcoming month. Tuition is considered late after the 5th and a late fee of $15.00 will be added to my account.
