Liability Release Form

I agree that if my child engages in any physical exercise or activity including but not limited to dancing, stretching, lifting, climbing, exercising, performing, turning, jumping, leaping, acrobatics, or gymnastics, or use any Mbodiment, LLC (“Studio”) facility on or off the premises, they do so at their own risk. This includes, without limitation, their use of the studio, locker room, changing room, parking area, sidewalk or any equipment in the studio and their participation in any activity, class, program or instruction, whether it involves the use of such facilities or not, and whether it is off-site during a competition, class or show. I agree that my child is voluntarily participating in these activities and using these facilities and premises and I assume all risk of injury, illness, damage or loss to themselves or their property that might result, including, without limitation, any loss or theft of any personal property. I agree on behalf of my child and their successors, personal representatives, heirs, executors, administrators, agents and assigns to release and discharge the Studio, and its owners, affiliates, employees, agents, representatives, successors and assigns, from any and all claims or causes of action, known or unknown, arising out of Studio’s negligence. This Waiver and Release of Liability includes, without limitation, injuries which may occur as a result of: (a) their use of any studio, dance or exercise equipment or facilities which may malfunction or break; (b) Studio’s improper maintenance of any studio, dance or exercise equipment or facilities, (c) Studio’s negligent instruction or supervision, and (d) their slipping and falling while in the studio or on the premises.


I ACKNOWLEDGE THAT I HAVE CAREFULLY READ THIS WAIVER AND RELEASE OF LIABILITY AND FULLY UNDERSTAND IT. I VOLUNTARILY ASSUME ALL RISK KNOWN AND UNKNOWN, FOR INJURIES OR DAMAGES, HOWEVER CAUSED, EVEN IF CAUSED IN WHOLE OR IN PART BY THE ACTION, INACTION, OR NEGLIGENCE OF STUDIO, AND ITS OWNERS, AFFILIATES, EMPLOYEES, AGENTS, REPRESENTATIVES, SUCCESSORS AND ASSIGNS, TO THE FULLEST EXTENT OF THE LAW.

Student Release Form
General Photography, Filming and Recording for Studio Use

Mbodiment Studios and Mbodiment LLC, collectively referred to as the “Studio”, will occasionally photograph, film and/or record their students for marketing or promotional purposes. Uses may include, but are not limited to:
Social media platforms and websites
Fliers and posters
Newsletters

This release allows your child to be photographed, filmed and/or recorded while at the Studio, at Studio-related events such as competitions and recitals, and any other situation where your child may be assumed to be a representative of the Studio.

I AGREE for my child to be photographed, filmed and/or recorded by representatives of the Studio, including but not limited to, teachers, choreographers, staff, and members of the Studio who are acting on the Studio’s behalf. I hereby grant the Studio the right to use my child’s photograph, video, and/or voice for use in all manner of media as described above. I understand and accept that the Studio is the sole owner of all right, title and interest under copyright in and to all recordings. I agree that I am not entitled to any additional consideration or compensation as a result of the rights granted herein.

If I DO NOT AGREE for my child to be photographed, filmed and/or recorded by representatives of the Studio for marketing or promotional purposes, I will EMAIL MBODIMENT STUDIOS AT contact@mbodimentstudios.com stating as such. I understand that my child may be photographed, filmed and/or recorded during rehearsals for the sole purpose of learning choreography for events such as competitions, recitals and other performances. These recordings will not be used for marketing or promotional purposes.

IF WE DO NOT RECEIVE A COMPLETED AND SIGNED RELEASE, YOUR CHILD WILL BE OPTED IN.
