You must be over 21 to register for an account and enroll in classes. Payment is due at the time of enrollment. All registered families are required to sign up for Auto-Pay in order to attend classes at JDF Elite Dance. All parents/guardians must agree to the registration waiver to proceed.
I understand and agree that JDF Elite Dance Academy and all staff and instructors are not liable for personal injuries or loss of property. Parent understands and expressly assumes all risks involved in connection with dance instruction, competition, and training at JDF Elite Dance Academy. This includes but is not limited to the risk of bodily injury which may occur as a result of contact with other students and/or equipment. Each student may decline to participate in an activity which may be harmful and is responsible to inform the instructor of any physical limitations which may prevent full participation in the class. I agree to be responsible for reading studio correspondence and respecting deadlines. I agree to wave the receipt of printed copies of all policy documents. I further give JDF Elite Dance Academy permission to use my child's likeness and name(s) for publication, advertising, or publicity. I am aware of the payment schedule set forth by JDF Elite Dance Academy and by signing agree to be responsible for said scheduled payments. I understand all tuition and fees paid are NON-REFUNDABLE and further understand and agree I must give 30 day notice to drop from classes in order to not continue to be charged for the following months. I have read the rules and regulations, dress code and calendar of JDF Elite Dance Academy and understand and agree that enrollment indicates acceptance and compliance of the program.
