I am the parent/guardian of the dancer(s) connected to me on this account, a minor, who is enrolled as a student in a course of instruction in dance with the studio, Relevé Dance Studio. By enrolling the minor in this course, I certify that I am aware of the possibility of injury/illness in connection with the activities, connected therewith. I understand and agree that neither the owners, operators, agents, or instructors, including but not limited to Dorothy Dixson, may be held liable in any ways for any occurrence in connection with the dance studio which may result in injury/illness or other damages to the minor, me or the minor's family heirs or assigns
In consideration of the minor being allowed to enroll in this course, I hereby personally assume all risks in connection with the course, and I further release the instructors and owners of any injury/illness or damage which may befall the minor while the minor is enrolled as a student of the studio, including all risks connected therewith, whether foreseen or unforeseen; and further, to save and hold harmless the program and persons from any claim by or on behalf of the minor or the minor's family, arising out of the minor's enrollment and participation in the course.
Further more I understand that Relevé Dance Studio and their respective directors and instructors assume no liability for lost, misplaced, stolen and/or damaged property and hereby agree to release Relevé Dance Studio from such liability.
I agree that I am financially responsible for the payment of monthly tuition, costume fees, any late fees that incur, and any other associated fees related to dancing at Relevé Dance Studio. I understand that there are no refunds, credits, or deductions for classes missed or discontinued or items purchased. I may, however; make up an missed classes within a one-month period. I understand that I may discontinue my classes and financial obligation to Relevé Dance Studio prior to the 1st of the month payment without any financial obligation.
I further state that I am of lawful age and legally competent to sign this affirmation and release that I understand that terms herein are contractual and not mere recital; and that I have signed this document as my free act.
