I agree to abide by all the rules of MKC Project, which is designed to create an orderly and comfortable environment for all students, parents and faculty of MKC Project.

I understand that I must pick up my child/children in a timely manner after their scheduled class has ended.

I agree that I will not leave my child/children unattended at the studio and fully acknowledge that the studio is not a daycare.

I understand MKC Project, LLC has the right to refuse class/performance to any student who is disruptive, not paid in full, or in violation of any policies/rules set by MKC Project, LLC.

In-person classes are closed for observation to provide a safe and distraction-free environment for the students. We will also adhere to the safety/health guidelines in place by our state and county when required.

Parents may drop off your dancer upstairs. At pickup time, parents are asked to wait in the courtyard to alleviate traffic and to ensure that our hallway/stairs area remains clear. Younger dancers will be escorted downstairs at the end of each class to meet parents at the courtyard door. Thank you in advance for your cooperation!

I release MKC Project, LLC DBA MKC Project, it's instructors, independent contractors, and all other associates from liability for harm, theft, or injury that may be suffered by me and/or members of my family traveling to or from or during participation in activities and programs sponsored by MKC Project, LLC.

I hereby acknowledge that I am voluntarily assuming full responsibility for all risks of physical injury arising out of active participation in a dance class or other dance related activities.

I understand that myself (or my student) must have this release form signed before attending class.

I acknowledge the contagious nature of COVID-19 and other contagious diseases and viruses and voluntarily assume the risk that I and/or my children may be exposed to or infected by COVID-19 by attending and participating and that such exposure or infection may result in personal injury, illness, permanent disability, and death.

I understand that the risk of becoming exposed to or infected by COVID-19 and other contagious diseases and viruses may result from the actions, omissions, or negligence of myself and others, including, but not limited to, employees, independent contractors, volunteers, and program participants and their families.

I release MKC Project, LLC DBA MKC Project, its instructors, independent contractors all associates from liability for harm, injury or death pertaining to COVID-19 and other contagious diseases and viruses.

I understand that there are NO refunds for registration fees, costumes, performance fees or tuition.

I understand that if my class gets cancelled due to weather, sickness or any issues arising from COVID-19, there is no refund for these missed classes.

I understand that myself (or my student) must have this release form signed before attending class.

I agree to wear a properly-fitted mask at all times when I am inside the MKC Project, LLC studio space. Should I be signing this release on behalf of my minor child, I agree to make sure they are properly masked at all times.

**As of 12/5/22, masks are optional, but strongly encouraged. Please discuss personal preferences and comfort level with your dancer. We will keep you updated should we transition back to requiring masks indoors.

I understand that MKC Project, LLC reserves the right to cancel any service, class or program at any time. Should this occur, MKC Project, LLC will refund your submitted payment.

I understand that recordings of myself or my child captured during class of MKC Project, LLC events via digital video or photos may be used solely for the purposes of MKC Project, LLC's promotional material and publications, and waive any rights of compensation or ownership thereto.
