By initialing here you give your permission for photographs/videos of the enrolled participant to be used for advertising/social media for CSDM. You will not be compensated for use of the photos/videos.

Covid-19 Waiver: We understand the risks associated with COVID-19 and the CDC guidelines for health & safety. I hereby release CSDM, its owner, officers, agents, employees, instructors, subsidiaries, and all affiliated entities from responsibility or recourse should we contract COVID-19.

Tuition is a monthly fee and does not change due to absences or cancellations. Tuition payments must be received on or before the 1st of the month. Any payments received after the 1st of the month will be subject to a $10 additional fee.

There are risks of physical injury associated with, arising out of and inherent to the activity of dance.
