Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19 The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. GSAB, llc has put in place preventative measures to reduce the spread of COVID-19; however, GSAB, llc cannot guarantee that yourself (your child) will not become infected with COVID-19. Further, participation could increase your child’s risk of contracting COVID-19. By signing this agreement, the undersigned acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that the undersigned may be exposed to or infected by COVID-19 by participation; and that such exposure or infection may result in personal injury, illness, permanent disability, or death. The undersigned understand(s) that the risk of becoming exposed to or infected by COVID-19 at GSAB, llc may result from the actions, omission, or negligence of the undersigned and others, but not limited to,GSAB, llc staff, volunteers, program participants, and their families. The Undersigned voluntarily agree(s) to assume all of the foregoing risks and accept sole responsibility for any injury the undersigned (including but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense of any kind, that the undersigned may experience or incur in connection with the undersigned’s participation at GSAB, llc. The undersigned hereby releases, covenant to sue, discharge, and hold harmless GSAB, llc, its staff, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. The undersigned understands and agrees that this release includes any Claims based of the actions, omissions, or negligence of GSAB, llc, its staff, agents, and representatives, whether a COVID-19 infection occurs before, during or after participation at GSAB, llc. The undersigned represents that they have adequate insurance to cover any injury or illness the undersigned may suffer or cause while participating in this activity or else the undersigned agrees to bear the costs of such injury or illness themselves. The undersigned further represents that they have no medical or physical conditions which could interfere with their safety in this activity, or else they are willing to assume – and bear the costs of – all risks that may be created, directly of indirectly, by any such condition. The Undersigned agree that the substantive law of Texas shall apply to the enforcement of this agreement and any portion of this agreement that may be found void or unenforceable, the remaining portions shall remain in full force and effect. By signing this document, The undersigned agrees that if the undersigned is exposed or infected by COVID-19 during their participation in this activity then the undersigned may be found by a court of law to have waived their right to maintain a lawsuit against the parties being released on the basis of any claim of negligence. By signing this document, the undersigned hereby agrees, represents, and warrants that neither the undersigned nor such participating children shall visit or utilize the facilities, services, and programs of GSAB, llc if he or she (1) experiences symptoms of COVID-19, including, without limitation, fever, cough, shortness of breath, loss of taste or smell, rash on hands or feet, or has a suspected diagnosed/confirmed case of COVID-19. The undersigned further agrees to notify GSAB, llc immediately if he or she believes that any of the foregoing access/use restrictions may apply. The undersigned has had sufficient time to read this entire document and, should I choose to do so, consult with legal counsel prior to signing, Also, the undersigned understands that this activity may not be available to them if they were to choose to not sign this release or that if the undersigned violates the terms of this agreement in anyway they may be asked to no longer participate in activities at GSAB, llc. The undersigned agrees to practice social distancing and clean hygiene during their participation at GSAB, llc. By signing below, I agree to each statement above and release Greater San Antonio Academy of Ballet, LLC from any and all liability for unintentional exposure or harm due to COVID-19.

Below is an outline of following requirements that should be followed by the dancer(s) and their parent(s).

General outlooks
• Student wears proper attire to classes, including hair secured in a bun, no pony tails! or loose hairs. Proper ballet shoes and tights (no holes)
• Student comes to class ON-TIME & consistency (good attendance)
• Student demonstrates a good attitude and able to reciprocate corrections

*Creative Movement: Listen and follow directions. Be able to retain simple sequences. Able to stand in line when asked. Understand first and second position and be able to follow arm and leg patterns. Not excessively asking to go to the bathroom. MAKE SURE TO POTTY BEFORE CLASS!

*Ballet/Tap 1: Understand first, second, third position. Be able to demonstrate arms (port de bras), tendu, passe (balance on one leg), be able to count at least to 8, skip across the floor, chasse front and side. Be able to remember the class curriculum. Pays attention and does not wander off to do own thing. Talking to a minimum. Understand the difference between heel, ball and toe. Shuffle Step patterns. Be able to buckle their own tap shoes.

*Combo 2: Demonstrate good listening skills. Understand all the position, and “en croix” patterns, musicality, flexibility, good posture. Shuffle patterns and flaps.

*Primary: Minimum 1 year at GSAB and understand the basics from previous year(s) (Ballet/Tap), simple combinations, musicality, coordination, flexibility ( be able to do the splits on both sides) .

*Ballet 1: At least 1 year of prior ballet experience with GSAB. Requirements: must be able to do full barre, extension (hold legs) at 90 degrees in all positions, be able to balance at least 5 seconds in retire. Be able to do clean double pirouettes with straighten knees and high relevé. All students moved up to the Level 2 wear pointe shoes. To be considered for pointe, dancer must have at least 1 year of pre-pointe followed by 2-3 weeks of Summer Dance. This doesn’t mean everyone will automatically get moved up. It’s contingent upon the students’ work ethic.

*Ballet 2: At least 2+ years of prior ballet experience with GSAB. Requirements: must be able to pick-up combinations, extension (hold legs) above 90 degrees with proper alignment, reciprocate corrections, know all the facings and terminology, know when to use their “épaulement” (use of head and upper body). understand how to beat in petite allegro. Understand the different "Facings".

*Pointe
• It is up to the teacher’s discretion when placing a dancer on pointe or the need to take dancers OFF pointe.
• How will I know my child is ready for pointe? All students moved up to the Level 2 wear pointe shoes. To be considered for pointe, dancer must have at least 1 year of pre-pointe followed by 2 weeks of Summer Dance Program. This doesn’t mean everyone will automatically be moved up. It’s contingent upon the students’ work ethic.

• What is the age requirement for getting pointe shoes? We recommend students to be at least 10-11 years of age and have completed a solid year of ballet 3-4 days per week. It is very important to know the dancer has established a solid foundation and understanding of the ballet technique before moving forward to prevent injury and wrong muscular development. Consistency in training is always key. The more classes, the better the results.
• Where do I get the pointe shoes? Your instructor will set a day to get you fitted at the local dance store.
• Dancers should arrive early to class to warm their feet with a thera band and work their feet daily to help increase their strength; practice is also encouraged at home.

• The registration fee $40.00 and one month of tuition is due upon registration
• Students whom owe tuition from previous month will not be allowed to participate until amount is paid.
• Tuition is due every 4 weeks ; you have a grace period of 5 days to pay tuition. If we do not receive tuition on that Saturday, you will be sent an invoice to be paid before the following Monday.
• $10.00 late fee will be added to your bill for every day missed.
• $10.00 if you lose your key tag and $2.00 for every time your child forgets to bring it to class.
• $30.00 fee for all returned checks
• ALL PAYMENT IS NON-REFUNDABLE! GSAB will do our best to offer make-up classes for your child due to absents, weather, etc. Limited to 1 make-up per month.
