Media Release Form

I, hereby grant Elite Performance Academy permission to use photographs, videos, and/or audio recordings of myself (or my child, if participant is under 18 years old) taken during dance classes, rehearsals, performances, or related events for the following purposes:

1. Promotional Materials: Including but not limited to brochures, flyers, posters, banners, websites, social media platforms, and advertisements to promote Elite Performance Academy and its programs.

2. Educational Purposes: Including but not limited to educational presentations, workshops, lectures, and demonstrations conducted by Elite Performance Academy or its representatives.

3. Archival Purposes: Including but not limited to maintaining historical records of Elite Performance Academy's activities and achievements.

I understand that the photographs, videos, and/or audio recordings may be used indefinitely by Elite Performance Academy and its affiliates, and I waive any right to inspect or approve the finished product, including written copy that may be created in connection therewith.

I hereby release, waive, discharge, and covenant not to sue Elite Performance Academy, its owners, instructors, employees, and agents from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to the use of the photographs, videos, and/or audio recordings, including without limitation any claims for invasion of privacy, defamation, or copyright infringement.

I certify that I am at least 18 years old and have read this media release form in its entirety and fully understand its contents. If the participant is under 18 years old, I certify that I am the parent or legal guardian of the participant and have the authority to consent to the terms of this media release form on their behalf.

Medical Waiver Form

I, understand that participation in dance and/or tumble classes at Elite Performance Academy involves physical activity and carries some risk of injury. In consideration of being allowed to participate in classes, I hereby agree to the following:

I acknowledge that I am solely responsible for monitoring my own physical condition throughout the class and will immediately report any discomfort, pain, or injury to the instructor.

I understand that it is my responsibility to inform the instructor of any pre-existing medical conditions, injuries, or physical limitations that may affect my ability to safely participate in class activities.

I understand that while the instructors at Elite Performance Academy strive to provide a safe environment, accidents and injuries can occur despite precautions. I agree to follow all safety instructions provided by the instructors and to use proper technique during all activities.

I hereby release, waive, discharge, and covenant not to sue Elite Performance Academy, its owners, instructors, employees, and agents from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or any property belonging to me while participating in classes or activities at Elite Performance Academy.

I understand that this waiver and release of liability is intended to be as broad and inclusive as permitted by the laws of the state/province in which Elite Performance Academy operates and that if any portion thereof is held invalid, the remainder shall continue in full legal force and effect.

I hereby grant permission to Elite Performance Academy to seek emergency medical treatment for me in the event of an injury, accident, or illness during class, and I agree to assume all costs associated with such treatment.

I have read this waiver and release of liability form in its entirety and fully understand its contents. I acknowledge that I am signing it voluntarily and of my own free will.
