General Liability Release Name of Student: __________________________________D.O.B.: __________ I, ____________________________________ (Name of Parent or Legal Representative), acknowledge that my daughter, son, and/or child for whom I have legal custody has voluntarily applied to engage in dance and dance-related programs and activities with NS Dance Academy (NSDA). I understand I should be aware of my child's physical limitations and agree to inform the studio of such limitations. I understand that participation in dance class, practice, workshop, camp, intensive, rehearsal, performance, competition or other dance activity is potentially risky. I knowingly and voluntarily agree to assume all reasonable risks and responsibilities for any such injury or accident, which might occur to my child or me before, during, or after any NS Dance Academy or dance-related activity that my child or myself has chosen to participate in except for those resulting from negligence or willful misconduct on the part of NS Dance Academy or any of its agents. I am aware that NS Dance Academy classes are given at the NS Dance Academy facility and/or , my child may on occasion use facilities or patronize businesses that are outside of the dance studio or be driven by an agent of NS Dance Academy between locations as a convenience to parents and I understand that NS Dance Academy is not responsible for any injury or accident that occurs under such circumstances unless it arises due to negligence or willful misconduct on the part of NS Dance Academy or any of its agents. I knowingly and voluntarily exempt, release, indemnify, and hold harmless Bay Area Dance School, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, independent contractors, and/or students from any and all liability claims, demands, or causes of action whatsoever from any damage, loss, injury, or death to me, my children, or property which may arise out of or in connection with participation in any classes, activities or events conducted by Bay Area Dance School, except for those resulting from negligence or willful misconduct on the part of NS Dance Academy. I further hereby knowingly and voluntarily agree to waive my rights and that of my heirs and personal representative(s) to hold NS Dance Academy, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, independent contractors, and/or students liable for such damage, loss, injury, or death.

I hereby give NS Dance Academy permission to use still or video images of my child dancing with NS Dance Academy in any reasonable form of advertisement for the studio.

I hereby give NS Dance Academy permission to seek emergency treatment for my child and I authorize NS Dance Academy representatives to give permission for the performance of emergency medical and dental examination(s) and necessary treatments (including tests, x-rays, medication or emergency procedures) as deemed necessary by the physician/dentist in attendance. If any emergency arises, NS Dance Academy representative will attempt to reach me and/or my child's emergency contacts and be guided by given instructions. If I cannot be reached, I authorize the attending physician/dentist to act as reasonable medical judgment dictates. If I would like to request that our doctor/physician be called and that my child be transported to a specific hospital, I am responsible for providing such information and physicians' phone numbers to NS Dance Academy representatives. I agree to accept financial responsibility for the costs/debts related to any and all emergency treatment deemed necessary by the physician/dentist in attendance. In the event that my child has a special medical need such as a life-threatening allergy or a disease which requires monitoring or medication, I will make appropriate arrangements in advance and provide all necessary information to NS Dance Academy.

I understand that there is a $35 non-refundable registration fee, per family, required yearly with the registration form and this signed policy paper to reserve a class space. Registration is not considered complete and class space is not reserved until this fee is paid and the registration and release forms are filled out. I also understand that tuition is due on the first of each calendar month. If accounts are paid after the tenth of the month, there will be a $35.00 late fee applied to the account balance. There is a $35.00 returned check charge for any checks returned by the bank. Tuition remains the same whether it is a long or short month and regardless of absences or cancellations. Classes will be pro-rated for holidays or cancellations but not for absences. Students can make-up a missed class within 4 weeks. If a student misses 6 consecutive classes, he or she may be dropped from the class for poor attendance. I understand that if my monthly auto payment does not go through, I will be contacted (phone/email) and a $35 late fee will be added. The balance must be taken care of before coming to the next class. If my card is lost or stolen, I will have a week to take care of the balance. I am also aware that no costumes will be ordered or sent home with outstanding balances. Notification of quitting must be in writing and received by the last day of the month by 6:00 p.m. or I will be responsible for the next month's payment, as my space has been reserved. I understand that Tuition is paid in 10 equal installments based upon the total numbers of classes for the year. December has fewer classes, whereas other months have more classes. The installments are the same each month regardless of the classes held. I understand if I do not properly cancel all of my classes within 6 months in advanced of the semester or other period of enrollment, I will remain responsible for paying tuition and fees assessed to my student account.
