Dancer's Dreamhouse Liability Waiver and Policies
Dancer’s Dreamhouse may revise or update policies at its discretion. Review policies and waiver at your leisure. Policies are subject to change.
Registration
All families must complete registration and sign waivers before beginning classes. Accounts must be current for a family to register for new classes. REGISTRATION FEES ARE NON-REFUNDABLE.
Tuition and Withdrawal
Tuition is divided over an 11-month dance season; August-June. Tuition is charged via autopay to a debit card or credit card on the 21st of the month prior. (This information can be updated anytime through the family portal.) Contact info@dancersdreamhouse.com to use another method of payment.
For your convenience, tuition is paid in equal monthly installments due on the 21st of the month prior, regardless of calendar days, inclement weather, or holiday closings. Tuition is not decreased for 3-week months or increased for 5-week months. Payment is considered late on the 23rd of the month, and a $20.00 late charge is added to all unpaid accounts. If payment is not satisfied by the 1st of the following month, your student will not be permitted to participate in classes until all tuition payments are up to date. Tuition is due even if the student misses a class to maintain the student’s place in the class.
Withdrawal from the program requires notice by the 20th and must be made in writing by email to info@dancersdreamhouse.com, or tuition will be due. A $40 early withdrawal penalty fee will also be incurred. All tuition fees are NON-REFUNDABLE.
Waiver
I release Dancer's Dreamhouse, its instructors, independent contractors, and all other associates from liability for harm, theft, or injury that may be suffered by me and/or members of my family traveling to or from or during participation in activities and programs sponsored by Dancer's Dreamhouse.
I hereby acknowledge that I am voluntarily assuming full responsibility for all risks of physical injury arising out of active participation in a dance class or other dance related activities. I acknowledge the contagious nature of COVID-19 and other contagious diseases and viruses and voluntary assume the risk that I and/or my children may be exposed to or infected by COVID-19 by attending and participating and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 and other contagious diseases and viruses may result from the actions, omissions, or negligence of myself and others, including, but not limited to, employees, independent contractors, volunteers, and program participants and their families. I release Dancers Dreamhouse, its instructors, independent contractors, and all associates from liability for harm, injury or death pertaining to COVID-19 and other contagious diseases and viruses.
I understand that fees are NON-REFUNDABLE. Full tuition refund may only be requested 24hrs before the start of the 1st class day of the season. There will be no refunds after the 1st day of class of the season is rendered.
I understand that if my dancer's class gets cancelled due to weather, illness, or any issues arising from COVID-19 there is no refund for these missed classes. Make-up classes will be provided if scheduling permits.
I give permission to Dancer's Dreamhouse to use pictures and or video of my child for advertising purposes in print, promotional videos, on social media, and on the company's website.
I understand and agree to Dancer's Dreamhouse's policies and procedures. I understand that my student must have this release form signed before attending class.
