POWERHOUSE TALENT (PHT)
PARENTAL/GUARDIAN LIABILITY WAIVER

I, individually or as parent or legal guardian of the dance student listed on the registration form, (participant). I understand that dance instruction, performance and related activities involves some risk of injury or harm, and I assume all liability (whether for injuries or otherwise) arising out of participant's involvement in any such activities. I agree to indemnify and to hold PHT and its owners, and employees, and its landlord harmless from and against any and all liability, damages, costs, claims, and expenses incurred by any of them in connection with participant's involvement in any activity on PHT premises. If PHT determines that an injury suffered by participant requires medical attention, I understand that PHT will make every reasonable effort to contact me. I hereby give permission for emergency medical treatment to be given to participant by providers chosen by PHT; however, if that effort is unsuccessful, PHT, its owners and employees will not be held liable. By marking the checkbox on the registration form, I further bind myself, participant, and our respective heirs, executors, administrators, successors, and assigns.

MEDIA RELEASE FORM
PHT uses the Internet as a tool to improve communication and to highlight dancer's successes. Individuals and groups will be showcased through the posting of writing, multimedia projects, photographs, videos, voice recordings and the listing of dancer's accomplishments on the company's website or other media outlets. I give my permission for participant's image, voice and achievements to be included on the PHT website or other media outlets.

FINANCIAL RESPONSIBILITY
I understand and agree that enrollment in Powerhouse Talent requires a level of commitment from me and my student dancer. I understand and agree to commit to at least one hour per week towards my child's dance education. I understand and agree that enrollment also involves a financial responsibility and will pay my balance in full on a monthly basis.

This page is used to inform website visitors regarding our policies with the collection, use, and disclosure of Personal Information if anyone decided to use our Service, the Powerhouse Talent website or Parent Portal.
