I understand and agree to the Auto-Pay policy.

By signing up for Auto-Pay, you understand and agree that any tuition due will be charged on the 1st of each month, your annual registration fee will be charged on the 1st of the month of your yearly "anniversary", and any signed for "charge my card on file" charges will charge the card enrolled in Auto-Pay. All charges are non-refundable.

I have read and understand the parent/student handbook, and agree to abide by the terms stated.

I understand that dance, as a physical activity, carries with it the risk of injury. I release West Jersey Youth Ballet, its teachers, and affiliates from any and all liability for injury, whether in or out of the studio, at a performance, convention, competition or any other studio activity.

I understand that photos and/or videos taken in classes may be used for promotional purposes in advertising and social media. In rare cases, first names may be posted for celebratory events.
