I am the legal guardian of student that I am registering.

Xplosive Dance Academy LLC will not be responsible for costumes once ordered from the company; there will be no refunds or exchanges. If you are planning on participating in June recital, a mandatory costume deposit of $40 per class will be due by October 15th. If this deposit is not made, a costume will not be ordered for your child. The remaining balance for June Recital costumes will be due no later than December 9th. Your remaining balance will be provided to you prior to the due date. We advise paying a little extra per month ($10/month/class) to aid with costume payment. These costume payments will be charged automatically with your card on file. We will send an email reminder before charging the card. Refunds are NOT administered to accounts for any reason. We can apply credit to an account if situation calls for it. Documentation must be provided, and then it is still in the discretion of the employees of Xplosive Dance Academy LLC.

Xplosive Dance Academy LLC is taking many precautions to help keep students safe and healthy, such as sanitizing common spaces, reminding students to wash hands, hand sanitizing stations, and more. We ask that you do your part by keeping your child home if they show any sign of illness, and alert the studio immediately if your child or anyone in the household tests positive for Covid-19. If a student comes into contact with someone with covid-19, they must wear a mask to the studio for the following 5 days. If a student tests positive for covid-19, they must stay home for 5 days and then wear a mask to the studio for the following 5 days. If a student is coughing or feeling sick at the studio we will require a mask to be worn until picked up.

Xplosive Dance Academy LLC is a "hands on" learning environment. To properly teach and demonstrate dance techniques physical interaction is often used. This is normal and expected in the teaching of dance. I parent/guardian understand this, and I give my consent for this type of teaching.

The undersigned Parent/Guardian or Participant hereby holds Xplosive Dance Academy LLC , all instructors, all business members, and/or all other organizations involved with the studio or performances harmless from any bodily injuries sustained while participating and/or while on the premises. It is also understood that Xplosive Dance Academy LLC will not be held responsible for the loss or damage of any personal articles or jewelry during performances or while on the premises. Xplosive Dance Academy LLC does all that it can to ensure all children are safe from injury, but should an emergency arise and you cannot be reached, your signature is required to give Xplosive Dance Academy LLC authorization to treat the child and/or call an ambulance if necessary.

Tuition will be paid every 8 weeks. An email reminder will be sent out a week prior to the due date. There will be a 3-day grace period in case of any problems, after that a $25 late fee will automatically be applied to your account. Once the fee is applied it cannot be removed! You are responsible for paying this on time. You must have a credit or debit card on file, this will be automatically charged on the tuition due date. If the card declines, you will receive an email and must remit payment within the 3-day grace period to avoid any fees. If a card on file declines more than 3 times, there will be a $15 penalty fee for every time it declines thereafter. You can change what card is on file with us at any time as well as access your transactions through our portal. Accounts will be charged every 8 weeks until the end of the season. If you choose to withdraw before the end of the season, you are required to email at least 2 weeks in advance to guarantee account will not be charged on the scheduled date. Refunds will not be granted for any reason, but in case of extreme circumstances, credits may be applied.

Xplosive Dance Academy has the right to use all photo and video that is taken during classes, performances, and any other event organized by the studio. These photos and or videos may be used, but are not limited to flyers, website, social media, and any other form of advertising/marketing that may present itself.
