I understand and agree that participation in any dance class, gymnastics class, workshop, performance, etc... poses a possibility for physical injury. I voluntarily agree, therefore, to assume all risk and responsibility for any such injury or accident which may occur to me or my child during any of In Motion Dance Company LLC's classes, workshops, performances, or activities. I also exempt and release In Motion Dance Company LLC , its owners, employees, assistants, volunteers, guest artists, and/or students from any liability claims, demands, or causes of action whatsoever from any damage, loss, injury or death to me, my children, or property which may arise from, or in connection with , participation in classes or activities conducted by In Motion Dance Company LLC. I voluntarily waive my right to hold In Motion Dance Company LLC, its owners, employees, assistants, volunteers, guest artists, and/or students liable for any such damage, loss, injury, or death. I understand that I should be aware of my physical limitations, as well as those of my child, and agree not to exceed them. I certify that I am the parent/legal guardian of the above listed child, and have the right to waive these rights.
I agree to have an emergency contact on file and agree to have a medical professional called if an emergency occurs.
