Waiver:

I hereby acknowledge that I am voluntarily participating in dance/martial arts classes offered by The Den School of Creative Arts. I understand that these activities involve a certain degree of risk, including but not limited to the risk of physical injury.

In consideration of being permitted to participate in these activities, I, on behalf of myself, my child (if applicable), my heirs, executors, administrators, and assigns, hereby release, discharge, and hold harmless the Studio, its owners, instructors, it's staff, LCA Leasing, and i'ts staff from any and all liability, claims, demands, actions, or rights of action, which are related to, arise out of, or are in any way connected with my participation or my child's participation in these activities, including but not limited to, any negligence of the Studio or its staff.

I understand and acknowledge that the Studio has provided a safe and suitable environment in which to participate in these activities. I agree to abide by all policies, rules, and regulations of the Studio, and I understand that any violation of these policies may result in my being asked to leave the Studio without refund.

I acknowledge that I have carefully read this waiver and release and fully understand its contents. I voluntarily sign this waiver and release, and I am aware that I am giving up legal rights that I might otherwise have.


Studio Policies:

Attire: All students are required to wear appropriate attire for their class. For dance classes, this includes shoes (that are specifically for the studio and not worn outside) and comfortable athletic wear that allows for freedom of movement. For martial arts classes, this includes comfortable athletic wear the allows for freedom of movement (students will be barefoot during class). Students may not wear any jewelry, except for small stud earrings.

Arrival time: Students are expected to arrive on time for their classes. Late arrivals may be asked to observe the class and may not be allowed to participate.

Respect: All students are expected to treat instructors, staff, and fellow students with respect at all times. Disrespectful behavior will not be tolerated and may result in the student being asked to leave the Studio without refund.

Cancellations: The Studio does not offer refunds for missed classes. In the event of studio closing due to unforeseen circumstances such as inclement weather, parents will be notified via email and/or text message and a notification will be posted on social media.

Parental supervision: Parents or legal guardians are responsible for the supervision of their child before and after class. Children may not be left unattended at the Studio.

Emergency contact: All students are required to provide a current emergency contact name and phone number in case of emergency.

By registering myself or my children for classes, I acknowledge that I have read and understand the Studio's policies and agree to abide by them.
