At Ink Cares and Dance Ink, we are following guidance to reduce the spread of COVID 19 and other illnesses. As in any other public place, we cannot guarantee that your child will not be exposed (while at Dance Ink or elsewhere) or if exposed, that your child will not contract an illness.
By signing below, you agree to waive any claims of liability against Ink Cares and Dance Ink LLC and to hold Ink Cares and Dance Ink LLC harmless if your child contracts COVID 19 or another illness despite our efforts to prevent it.
Additionally, I agree to notify Ink Cares/Dance Ink if my child or any member of our household contracts COVID 19 or has come into contact with someone who tested positive for COVID 19. This action will help ensure the utmost safety of our dancers and staff.
If your child has experienced any symptoms such as fever, cough, shortness of breath, chills, runny/stuffy nose, sore throat, diarrhea, vomiting, loss of smell or taste, or headache, we ask that the dancer stay at home for at least 24 hours after no longer having these symptoms.

Mandates will be instituted should the local or state government issue these and modifications made as necessary.

This form signifies my release of right to any and all photographs, audio and/or video clips that Ink Cares or Dance Ink has taken of me and/or my child that is a student.
I give Ink Cares and/or Dance Ink permission to edit and use photographs, audio and/or video clips of my child for purposes including, but not limited to, promotional and educational materials. These materials may be used in all forms of media, including print and digital.
I understand that these materials may be distributed to other individuals and/or companies, at which point Ink Cares and/or Dance Ink will no longer have control over their use. In addition, I release Ink Cares and Dance Ink from all liability related to the use of these materials.
I hereby relinquish rights to these materials.

By signing this Waiver and Release of Liability (Agreement), I waive and release Ink Cares and Dance Ink LLC, its agents, servants, employees, insurers, successors and assigns from any and all claims, demands, causes of action, damages or suits at law and equity of any kind, including but not limited to claims for personal injury, property damage, medical expenses, loss of services, on account of or in any way related to or growing out of my presence or involvement at the facility.

This waiver and release is intended to and does release Ink Cares and Dance Ink LLC from any and all liability for damages or injuries on account of or in any way related to or growing out of my negligence, the negligence of third parties and Ink Cares and Dance Ink LLC's negligence. This is not intended to release Ink Cares and Dance Ink LLC from any liability resulting from their intentional conduct. I further covenant and agree not to institute any claims or legal action against Ink Cares or Dance Ink LLC for any claim released by this Agreement. I further agree that should any claim be made against Ink Cares or Dance Ink LLC in contravention of this Agreement, including but not limited to derivative claims, I will protect, defend and completely indemnity (reimburse) Ink Cares and/or Dance Ink LLC for any such claim and expenses including attorney's fees and costs incurred by Ink Cares and/or Dance Ink LLC in defending themselves or security indemnity hereunder.

I understand that Ink Cares and Dance Ink LLC are not responsible for any lost, stolen, or damaged valuables or property.

I acknowledge that I have received and read a copy of the current rules and regulations governing the use of the facility. I agree that I will fully comply with all rules and regulations and with any amendments.

I have read the Agreement and understand that by signing the Agreement I have consented to be bound by its terms, including the waiver/release of any legal right I may have to sue Ink Cares and Dance Ink LLC for any costs they incur because a claim or legal action is brought in violation of this Agreement. I agree any violation of the Agreement and its terms and conditions, as determined by Ink Cares or Dance Ink LLC, will void and terminate this Agreement and may result in loss of the ability to use the facility.

Parent Video Release Agreement

I understand that I have enrolled my child or children at Dance Ink/Ink Cares
located at 225 W Main St, Carnegie, PA 15106, also referred to herein as the “Childrens Activity Center”. The Childrens Activity Center has a program whereby webcams are in use and my children are under streaming video surveillance that used for training and management purpose, in addition to, secure accessibility from the web in accordance with the terms and conditions associated with the Spot TV website (also referred to herein as “Spot TV”). By my signature below, I hereby consent to the photographing of myself and/or my minor child(ren)
and the recording of my voice and or that of my child(ren) named above. I agree that I have no rights to any of the video footage or photographs for any reason at any time. I also agree that I will not screenshot, copy, reproduce, alter, modify, or create derivative works from the Content and Service. I understand that unauthorized recording, duplication, or distribution of this copyrighted work is illegal. I assume full liability of all the terms outlined in this release
for any family member’s account activity associated with my child or children. Copyrighted work includes all web streaming and video/audio recordings. I understand that legal action can be taken against me by Spot TV or Children’s Activity Center for such copy right infringement. I understand that the term "photograph" as used herein encompasses still
photographs, audio, and motion picture footage.
