**I understand the activities and risks involved in these dance classes. I hold harmless, Prestige Academy of Dance, LLC, all employees and representatives from all liability, suits and claims arising from participation in or observation of any Prestige Academy of Dance, LLC, activities or events. Prestige Academy of Dance, LLC, is not responsible for any lost, stolen or damaged property. I also recognize Prestige Academy of Dance, LLC, is not responsible for students when they are outside the classroom.

**A Returned Check Fee of $30.00 will be posted for any checks that are returned.

**Tuition balance due the 1st of every month. A late fee of $15.00 will be charged to your account for all payments received after the 7th. of the month. This fee is automatically generated and cannot be reversed.

**By registering for this program, you give permission to take and publish photos of your child in print or online promotional purposes. If you do not wish to have your child photographed, you must include a request in writing. Please bring your document to the suite 1 receptionist to confirm receipt of your request.

** I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing. I further acknowledge that Prestige Academy of Dance, LLC has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19. I further acknowledge Prestige Academy of Dance, LLC cannot guarantee that I/Student/Family/Friend will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself/student/family /friend. I acknowledge that I/student/family/friend must comply with all set procedures to reduce the spread while present at Prestige Academy of Dance, LLC. All must attest that: * I/Student/Family/Friend are not experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell. * I/Student/Family/Friend have not traveled internationally within the last 14 days. * I/Student/Family/Friend have not traveled to a highly impacted area within the United States of America in the last 14 days. * I/Student/Family/Friend do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19. * I/Student/Family/Friend have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non-contagious by state or local public health authorities. * I/Student/Family/Friend am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19. We hereby release and agree to hold Prestige Academy of Dance, LLC harmless from, and waive on behalf of myself, student/family/friends, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act.. I understand that this release discharges Prestige Academy of Dance, LLC from any liability or claim that We heirs, may have against the studio with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Prestige Academy of Dance. This liability waiver and release extends to the Prestige Academy of Dance, LLC together with the owners, employees and family members.
