I, hereby agree to release, waive, discharge, and covenant not to sue Vensko Shively, LLC, doing business as Synergy Dance Collective, its officers, directors, employees, agents, volunteers, and affiliates (collectively referred to as "Synergy Dance Collective") from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me, or to any property belonging to me, while participating in any dance class, workshop, event, or any other activity organized or conducted by Synergy Dance Collective, whether caused by the negligence of Synergy Dance Collective or otherwise.

I am fully aware of the risks and hazards inherent in dance activities, including but not limited to physical exertion, loss or damage of personal property, and risks associated with physical contact with other participants or objects. I hereby assume all risks and hazards associated with my participation in any dance activities with Synergy Dance Collective, including but not limited to risks of physical or emotional injury, paralysis, death, or property damage.

I further hereby agree to indemnify and hold harmless Synergy Dance Collective, its officers, directors, employees, agents, volunteers, and affiliates from any loss, liability, damage, or costs, including court costs and attorney fees, that may be incurred by Synergy Dance Collective as a result of my participation in any dance activities with Synergy Dance Collective, whether caused by the negligence of Synergy Dance Collective or otherwise.

I understand that this waiver and release applies to all dance activities organized or conducted by Synergy Dance Collective, including those not specifically named in this waiver and release.

I have carefully read and fully understand the contents of this waiver and release. I am aware that by signing this waiver and release, I am giving up certain legal rights, including the right to sue Synergy Dance Collective for any injuries or damages I may sustain while participating in any dance activities with Synergy Dance Collective.

I, hereby grant Vensko Shively, LLC, doing business as Synergy Dance Collective, its officers, directors, employees, agents, volunteers, and affiliates (collectively referred to as "Synergy Dance Collective") the right to use and publish any photographs, videos, audio recordings, or other media of me taken or recorded during any dance class, workshop, event, or any other activity organized or conducted by Synergy Dance Collective (collectively referred to as "media"), for any lawful purpose, including but not limited to marketing and promotional purposes.

I understand that Synergy Dance Collective may use and publish the media in various forms of media, including but not limited to print, electronic, online, and social media platforms.

I hereby release and discharge Synergy Dance Collective, its officers, directors, employees, agents, volunteers, and affiliates from any and all claims and demands arising out of or in connection with the use of the media, including but not limited to any claims for invasion of privacy, defamation, or copyright infringement.

I acknowledge that I will receive no compensation for the use of the media.

I am at least 18 years of age, or I am the parent or legal guardian of the minor whose image or voice is being released.

I have carefully read and fully understand the contents of this waiver and release. I am aware that by signing this waiver and release, I am giving up certain legal rights.
