Whilst every reasonable measure is taken to ensure the wellbeing of your child whilst in our care, in the event of an accident or illness requiring medical attention every effort will be made to contact you. If we are unable to contact you, we need your permission to respond to emergency situations and seek medical attention as required. I hereby authorise Move Dance Co teachers to consent, where they are unable to contact me, to my child receiving such medical treatment as may be deemed necessary. I understand that no liability will be taken for loss or damages of personal property or any physical injury caused whilst at Move Dance Co studios or any theatre and external performances/ rehearsals. Move Dance Co requests that you ensure that your child is under adult supervision at all times in the foyer area and that Move Dance Co is not responsible for your child on site outside of enrolled class times. PHOTO AND PROMOTIONAL RELEASE I agree for my child to have their photograph/video taken and used for promotional purposes of Move Dance Co. This includes their full name to be displayed in the newsletter, website, information booklets, concert programs and any other advertising material related to the company including all social media platforms such as Facebook and Instagram.
