Hearts in Motion LLC Waiver

Please read the waiver completely before signing.

I recognize the potential for risks and hazards when participating in dance class activities. I freely accept and fully assume responsibility for all such risks, dangers, hazards, and the possibility of personal injury, property damage, or loss resulting from all activities my child participates in.

I voluntarily agree to release Hearts in Motion LLC, its employees, and volunteers from any and all liability for any loss, damage, injury, or expense that my child or I may suffer as a result of participation in classes.

If I cannot be contacted, I authorize Hearts in Motion LLC to seek medical services in case of injury or serious illness.  I further agree to accept financial responsibility in excess of the benefits allowed by my health plan.

Photo/Video Permission

I give Hearts in Motion LLC permission to use my child’s photograph for advertising purposes on Facebook, flyers, and their webpage.
