NEW STUDENT WAIVER AND RELEASE FORM
Liability Waiver:
I, the undersigned, hereby acknowledge that I have voluntarily enrolled my child, _____________ (student's name), in dance classes at Inspirations Dance Studio. I understand that dance training may involve physical movement that carries a risk of injury. I agree to assume full responsibility for any injury my child may suffer while participating in any activities at Inspirations Dance Studio, whether on or off the premises.
I understand that Inspirations Dance Studio and its instructors will take all reasonable precautions to ensure a safe environment for students. However, I agree to release and hold harmless Inspirations Dance Studio, its employees, instructors, and representatives from any liability, claims, demands, actions, or causes of action arising out of or related to any injury or illness that may occur.
Medical Release:
In the event of an emergency, I authorize Inspirations Dance Studio and its staff to seek medical treatment for my child if necessary. I agree to be responsible for any medical expenses incurred. I understand that every effort will be made to contact me before medical care is administered.

Media Release:
I grant permission for Inspirations Dance Studio to take photographs and videos of my child during classes, performances, and other events. I understand that these images may be used for marketing, promotional materials, social media, and the studio’s website.

Payment & Refund Policy:
I understand that all tuition fees must be paid on time and that there are no refunds for missed classes, holidays, or cancellations. If my child withdraws from a class, I understand that I must provide written notice at least 30 days in advance and that any paid tuition is non-refundable.
Responsibility for Payment:
Inspirations Dance Studio charges an annual, non-refundable registration fee starting at $30 (as of July 2023), with a discounted additional fee for siblings. This fee is charged when you register. The fall session runs from September 16-December 14th for 13 weeks. There is no pay-per-class rate for self-pay families. No monthly fees will be prorated for holiday breaks.
Families using DDA, other contractors, and those on scholarships are still required to pay the registration, recital and costume fees unless your scholarship has been granted as a full year scholarship that includes ( registration fee, costume fees, and recital fees).
Tuition not paid by the 5th of each month will incur a $10 late fee. There is a $25 fee for all returned checks.
If a class is canceled due to weather or other circumstances, we will hold an alternate or make-up class. If your child misses class due to illness, tuition will not be prorated, but they may attend a make-up class. No refunds will be provided under any circumstances.
Any and all shows will include additional fees, including costume costs. Our Winter Show details are forthcoming. Any student participating in the fall program will have the opportunity to perform in our Winter Show. More information about this process will be provided closer to the event.
Class schedules are subject to change. Any class with insufficient enrollment will be canceled after the first two weeks. We often add classes and move students to different levels throughout the year to ensure that each student is in the best class for them. Please leave a message with the studio if your child is absent.
Non-Discrimination Policy:
Inspirations Dance Studio is committed to providing an inclusive, safe, and supportive environment for all students, staff, and families. We do not discriminate on the basis of race, color, religion, sex, gender identity, sexual orientation, national origin, age, disability, or any other characteristic protected by law. All students are welcome and encouraged to participate regardless of ability or background.
Procedure for Handling Discrimination Concerns:
Reporting: If you believe you or your child has been subject to discrimination or harassment, please contact the studio director immediately via email or in person. A formal written complaint may also be submitted.
Investigation: Upon receiving a report of discrimination, the studio will conduct a confidential and thorough investigation. This may involve interviewing the individuals involved and any witnesses.
Resolution: If it is determined that discrimination occurred, appropriate corrective actions will be taken, which may include disciplinary actions up to and including dismissal from the studio, depending on the severity of the situation.
No Retaliation: Retaliation against individuals who report discrimination or participate in an investigation will not be tolerated. Any incidents of retaliation should be reported immediately to the studio director.
