Registration fees are $60 per class. This fee includes a $50 costume deposit that will be applied toward the cost of the costume for the June production. Dancers who withdraw before the June production will not receive refunds for registration fees or costume deposits and payments.

I agree to keep a valid credit card on file in my Dance Studio Pro account and enroll in the automatic/recurring payment plan. Tuition is posted to accounts on the 1st of each month. Automatic payments are charged to the card on file on the 9th of each month. A $20 late fee is applied to overdue accounts after the 9th of each month.

I understand that Lee Anne Stewart Dance School does not give credit and/or refunds for classes missed due to holidays, vacations, illness, weather, etc. I am required to give a 30-day notice if I select to withdraw from classes.

In case of default of payment, I agree to pay any Legal Interest on the balance due together with any collection cost, reasonable Attorney Fees, and Court Costs incurred to collect this account.

I understand dance and related activities involve risk of injury. I agree I will not hold Lee Anne Stewart Dance Schools, Inc. (it's owners, staff, and related parties) responsible for injuries/damages incurred by any of my family members while participating or visiting the facility. In case of injury or illness, while participating, I authorize medical care for my child and accept responsibility for medical expenses.

The school is hereby granted permission to take photographs and videos to use in brochures, websites, social media, posters, advertisements, and other promotional materials the school creates. Permission is also hereby granted for the school to copyright such photographs in its name.

I have had sufficient opportunity to read this entire document. I have read and understood it, and I agree to be bound by it's terms.
