I understand that parents, legal guardians of minor students and adult students waive the right to any legal action for any accident or injury sustained on or around school property resulting from normal dance activity or any other activity conducted by the students before, during, or after class time. I understand dance is a physical activity, and as such there is inherent risk of injury ranging from minor to catastrophic, despite Elevate Dance Center's careful attention to safety and safe practices. I accept this risk and consent to dance classes with EDC.

I further understand EDC is not responsible for providing before or after class care for students. Students are not to be left at the school for excessive time before or after class. EDC is housed in a mall, and the staff is not responsible for the supervision of my dancer outside of your building. EDC will not leave dancers under the age of 18 waiting outside at night after closing time, even if I allow it in my family. If I am late picking up my dancer, and the staff is waiting with them after hours, then I understand I will be billed at a rate of $1 per minute and I consent to this billing.

As with the transmission of any communicable disease like a cold or the flu, I know I may be exposed to COVID-19, also known as "Coronavirus," at any time or in any place. Despite EDC's careful attention to cleanliness, hand-washing and personal space, there is still a chance my family could be exposed to an illness in the school just as we might be at the gym, grocery store, or our favorite restaurant. Due to the nature of the services provided, we know it is not possible to maintain social distancing at all times. Although exposure is unlikely, I accept the risk and consent to dancing with EDC.

In an effort to NOT increase prices, we have decided to pass credit card processing fees along to the client. You can choose to avoid the conveniences fees by paying in person with exact cash or check BEFORE the due date. The fees are exactly equal to what we pay (2.99% + .30 per transaction), and are not marked up. Thank you for your understanding and continued support.

I have chosen to enroll in a school with a dress code, and I will honor it. I understand required dancewear and shoes must be worn to all classes. Failure to comply with dress code may result in the student to sit out. I will not publicly share on any platform any choreography by Elevate Dance Center, LLC via video or by live performance without the express written permission of EDC. I understand this is intellectual property and I will not steal it.

I will hold harmless EDC and all staff. I knowingly accept this risk and consent to dance classes with EDC.

I understand missed classes do not roll over. No refunds are given for missed lessons or closures due to inclement weather. School activities such as trips, sports or other programs and school district holidays or breaks that differ from dance closures for holidays or breaks do not qualify for make-up lessons.

I understand policies are subject to change at any time. If there are changes, I will be notified in writing.

I hereby authorize Elevate Dance Center, LLC to electronically debit my payment on file for all monies owed. I understand I am responsible for the credit card fees associated credit payments. These are listed as "convenience fees," and are equal to the amount EDC is charged to process the payment. I understand I can always pay with another method not on file (such as exact cash or check to avoid convenience fees or a different credit card) as long as I do so BEFORE the due date. Otherwise, I know and fully authorize that my saved payment method will be charged on the due date. I understand all payments not received within 7 days of the payment due date will incur a $15 late fee. I understand returned payments and/or declined charges will incur a $15 NSF fee. I hereby authorize Elevate Dance Center, LLC to electronically debit my saved payment method for payments, including late or NSF fees when applicable.

I hereby grant the school permission to take photographs of the students to use in brochures, web sites, posters, advertisements, social media and other promotional materials the school creates. I also grant permission for the school to copyright such photographs in its name. I understand my child's name will never be published on a social media platform.

After you submit your registration, we will run your $25 per student non-refundable deposit at our earliest convenience in order to reserve your spot . When you make your first payment, you will see your $25 again as a credit to your account.

I understand Elevate Dance Center is not responsible for providing before or after class care for my student. Students are not to be left at the school for excessive time before or after class. Students under age 8 must be accompanied by an adult when entering the building. Students under 12 must be accompanied by an adult to exit the building. Students 13 and older may come and go as necessary, however EDC is not responsible for student behavior outside of EDC. I understand Staff will not leave dancers waiting for a ride alone after closing, even if allowed in my family. If I am late picking up my dancer, then I will be billed at a rate of $1 per minute on my next statement.

A trial class is $18. Enroll, and the $18 is credited back to your account. A trial class can be rescheduled up to 2 times with advance notice, however there are absolutely no refunds on trial classes, regardless of whether or not you attend the class.

I understand all tuition installments are due prior to services rendered on the 15th of the month during Fall Session- 8/15, 9/15, 10/15 and 11/15 or on the 1st of the month during Spring and Summer Sessions 1/1, 2/1, 3/1, 4/1 (spring) 6/1 and 7/1 (summer). If the tuition due date falls on a Saturday or Sunday, my saved payment method may be charged on the next business day. There is a student tuition cap of $390.25 per month. There is an annual school membership fee of $25 per student (not to exceed $65 total), due upon registration for new students or annually in August for current students. There is no tuition or membership fee for summer-camp only attendees.

If I choose to withdraw my student, I will give 14 days written notice via email. If I have completed the withdrawal process and have paid for services in advance, then I will receive a refund for unused services LESS all credit card processing fees (if applicable). If I do not want to pay my credit card processing fees upon withdrawing from class, I understand I may choose to receive a credit towards the next session's classes.

I opt in to all communications and will read email and log into the Parent Portal regularly. I know it is my responsibility to keep informed by reading the weekly 6am Friday email newsletter and by joining the BAND App because EDC is paperless. I will keep my contact information up to date with the school. I understand it is my responsibility to be aware of all important dance dates.
