Waiver of Liability: I, the undersigned parent or legal guardian of the dancer(s), do hereby give permission for the aforementioned persons to participate in any and all classes, practice, programs, shows and events offered by or attended by Lisa's Dance Spot. I accept all risks associated with that participation and understand that there is a full possibility of serious physical injury or illness. I hereby covenant not to sue and waive, release and forever discharge any and all rights and claims for damages, which may arise now or in the future against Lisa's Dance Spot and it's officers, owners, directors, employees and/or other assigned representatives or volunteers from any and all liability and for any and all damages and/or injuries which may be sustained or suffered by the dancer(s) while participating at or for Lisa's Dance Spot. Furthermore, I hereby give my permission to Lisa's Dance Spot to use photographs and/or video of the dancer(s) as deemed appropriate for the promotion of Lisa's Dance Spot. Last, ALL FEES ARE NON-REFUNDABLE. This includes registration fees, tuition fees, and recital fees. Monthly tuition fees are due regardless of the number of classes attended. Tuition is due on the 1st of each month and late if payment is received after the 7th of the month. A late fee penalty of $15 is applied if balance is not paid by the 7th of the month. Payment can be made online via the Parent Portal. You may also make payment inside of the studio during business hours. Any account showing a balance as of the 10th of the month will also receive a final reminder to make payment. Students will not be allowed to attend class until the tuition is paid in full at that time. This means, if there is a tuition balance outstanding on the 10th and thereafter, the student must not attend class until the balance is paid. Students will be deactivated from the program for nonpayment of tuition fees and will need to register again if payment is not made by the last day of the month. This means that if the tuition payment is still not paid by the last day of the month, then the student will be deactivated from the program and you would need to register all over again which also means you would be paying another registration fee. No balance carryovers will be allowed which means account balances may not be carried over into the following month. We will not place holds on any available class space to allow for others to register. Proper notification must be given when deciding to drop classes or tuition fees will continue to be charged.
