I understand that Cody Center Enterprises, INC (DBA Cody Center for the Performing Arts and Cody Center for Gymnastics and Acrobatic Arts) and The Studio Theatre @ CCPA does not give credit and/or refunds for class(es) missed due to holiday, vacation, illness, weather, etc. I further understand that there are specific risks of physical or property damages, losses, or injury that may result from my or my child’s participation with Cody Center Enterprises, INC and The Studio Theatre @ CCPA, and I voluntarily assume the risks associated with such participation.
I hereby consent to the participation in interviews, the use of quotes, and the taking of photographs, movies or video tapes of my or my child .I also grant to the right to edit, use, and reuse said products for non-profit purposes including use in print, on the internet, and all other forms of media. I also hereby release the Cody Center Enterprises and The Studio Theatre at CCPA from all claims, demands, and liabilities whatsoever in connection with the above.
On line payments, and auto billing are done through DanceStudio-Pro. Cody Center Enterprises, INC will have your credit, debit card or ACH bank transfer securely on file will automatically withdraw tuition by the 5th of the month or at the time of registration for summer camps and classes. If using a credit or debit card you will be charged an additional convenience fee for processing. If you wish to pay by cash or check you must do so before the 5th. All families must store a card or bank number on file.
RELEASE AND WAIVER OF LIABILITY AND INDEMNITY AGREEMENT
I understand that all of the activities of the program involve some degree of risk of bodily or personal injury to my child and loss or damage to personal property, and that my child will be one of several under the supervision of agents or employees of Cody Center Enterprises, INC and Studio Theatre at CCPA.
In consideration of my child’s participation in the program, I hereby freely and expressly assume and accept any and all risk of bodily or personal injury to my child, or loss of or damage to personal property, during or arising out of my child’s participation in the program. I hereby release Cody Center Enterprises, Studio Theatre @ CCPA it’s owners, agents and employees, from any and all liability for bodily or personal injury to my child, and for the loss or damage to personal property, which may occur during or arising out of his/her participation in the program. I further agree to waive any claims or cause of action that I may have, and to indemnify and hold Cody Center Enterprises, INC and Studio Theatre @ CCPA, its owners, agent, and employees, harmless from and against any and all claims, demands, causes of action, losses, damages or expenses related in any way, directly or indirectly, to my child’s participation in the program.
I hereby acknowledge that I have read this document carefully and that I fully understand the contents.
