Risk Awareness
We, the staff at Curtain Call Studios, recognize our obligation to make sure our students and their parents are aware of the risks and hazards involved in the performing arts taught at CCS. By signing, you understand the risk involved and are willing to participate and/or are willing to have your child participate. By signing, you release, indemnify, and hold harmless Curtain Call Studios and all it's employees from all claims and liability on account of any injury which may be sustained by you or your child while attending a performing arts class, lesson, event, or outside performance.

COVID-19
Curtain Call Studios cannot prevent you [or your child(ren)] from becoming exposed to, contracting, or spreading COVID-19 while utilizing Curtain Call Studios’s services or premises. It is not possible to prevent against the presence of the disease. Therefore, if you choose to utilize Curtain Call Studios’s services and/or enter onto Curtain Call Studios’s premises you [or your child(ren)] may be exposing yourself to and/or increasing your risk of contracting or spreading COVID-19.
ASSUMPTION OF RISK: I have read and understood the above warning concerning COVID-19. I hereby choose to accept the risk of contracting COVID-19 for myself and/or my children in order to utilize Curtain Call Studios’s services and enter Curtain Call Studios’s premises. I understand that this waiver means I give up my right to bring any claims including for personal injuries, death, disease or property losses, or any other loss, including but not limited to claims of negligence and give up any claim I may have to seek damages, whether known or unknown, foreseen or unforeseen.

By signing, you affirm that you now have and will continue to carry proper primary medical, health, hospitalization, and accident insurance which you consider adequate for the protection of both your child and Curtain Call Studios. You affirm that you or your child (whomever is the CCS student) is in proper health to participate in physical activity involved in CCS classes. You agree to inform CCS of any health issues or limitations. In case of emergency and the parent/legal guardian cannot be reached, you authorize all medical treatment and procedures to be performed by the attending physician and/or paramedics.

Media
By signing, you give CCS permission to take photographs or videos of you or your child (whomever is the CCS student), and use the photographs in brochures, web sites, social media, advertisements, promotional materials, or any other place that CCS sees fit.
