I confirm that I have read and understand all conditions and aspects of this waiver and acknowledge and agree that I and my enrolment is subject to the terms and conditions set out therein. I understand that dance tuition is a monthly amount that will be automatically debited on the 1st of each month (excluding January and December). I understand that I must provide at least 7 days notice if I wish to withdraw my child. I understand that if I withdraw my child, I will not receive any costumes or a USB. I understand that dance classes may include, without limitation, dancing with props, stretching, barre work, across the floor combinations, dance routines in the centre, and other related activities. I further understand that all of the activities of the dance class involve some degree of risk of strain or bodily injury. I further understand that I am responsible for ensuring my child is/children are, medically fit for the class(es).

I hereby agree that, in the advent of an accident or illness, I authorise for urgent medical assistance or treatment for my child(ren) including the calling of an ambulance.

I have read the 2025 Information Handbook and agree to adhere to all the content stated therein including:

*Compulsory Dress Rehearsal/Concert Dates

I agree to be responsible for reading studio correspondence and respecting deadlines, if applicable.

I also hereby give Dazzle Dance Academy and its staff permission to use and publish images (being photographs or film) taken of my child, children or myself for the purposes of advertising and promotion of the Academy. This includes, but is not limited to, the Concert USB and Programme, general advertising materials and online mediums such as the Dazzle Dance Academy website, the Dazzle Dance Academy Facebook page, and any other advertising material that the Academy wishes to use. The permission given herein is irrevocable. I am aware that by giving this authority Dazzle Dance Academy and its staff may continue to use the images of my child, children or myself beyond the date which my child is/children are enrolled at the Academy for advertising and promotional purposes.

I understand that Dazzle Dance Academy holds intellectual property rights to any solo, duo or group routines.
