You must be the dancer's caregiver to set up an account.

I understand and agree that participating in any dance class, workshop, rehearsal or performance carries risk, including but not limited to illness, injury or death. I certify that my child is able to safely participate in Bethesda Dance Center, LLC activities and I agree to assume all risks associated with my child's participation. I release Bethesda Dance Center and all associates from liability for harm, injury or death.
I understand the contagious nature of COVID-19 and other contagious diseases, and voluntarily assume the risk that my child and I may be exposed to or infected by COVID-19 by attending Bethesda Dance Center activities and that such exposure or infection may result in personal injury, illness, disability, and death.
I understand that it is the parent or guardian’s responsibility to drop-off and pick-up my child for all classes and performances. Failure to pick my child in a timely manner from class can result in fees. I acknowledge there will be no refunds for registration fees, missed classes or costumes.
I understand that photos and videos taken of my child are the property of Bethesda Dance Center, LLC, and I authorize Bethesda Dance Center, LLC to use photos and videos of my child for promotional purposes.
I have carefully read this Waiver & Liability Release and fully understand its contents. I understand the inherent risks in dance and release Bethesda Dance Center, LLC of all liability. I sign this Release on my own free will.
