I understand and acknowledge that participation (and/or my child's participation) in dance, yoga, and other physical exercise classes and activities is risky and could result in personal injury to myself (and/or my child) and to others. I also understand that accidents and injuries may occur. By signing this liability form, I assume all risks on behalf of myself (and/or my child) related to any and all classes and activities facilitated by Frame Dance Productions.

I agree to release and hold harmless Frame Dance Productions and its teachers, dancers, staff members, board members, independent contractors, and agents, as well as any and all facilities used by Frame Dance Productions, from any causes of actions, claims, demands, damages, and/or liabilities resulting from or relating to any personal injury to myself (and/or my child) or to others, or resulting from or relating to any damage to personal property, whether arising before, during, or after any activities, classes, dance workshops, and/or events facilitated by Frame Dance Productions. I further agree to obey all rules (and/or to ensure that my child obeys all rules) provided by Frame Dance Productions and agree to take full responsibility for my behavior (and/or my child's behavior).*
