2024-2025 DANCE SEASON
School Policies, Contract, Acknowledgements, and Waiver and Release Agreement
I have read and agree with Morton Street's policies and procedures including:
***Sampler classes at Morton Street Dance Center are not part of the 10-month contract and are offered as standalone sessions.
10 MONTH PROGRAM:
Once you are registered, you are responsible for your full accrued and unaccrued tuition for Morton Street Dance's ten-month program and any other registration related charges, costumes, or late fees assessed on your account; there are no refunds. In the event, that the student withdraws before the expiration of the 10-month term, you will be responsible to pay the remainder of your account balance (including accrued and unaccrued tuition, costumes, and other fees applied) within 30 days of your withdrawal date; after this time, outstanding balances are subject to collections.
I understand that children’s classes are a 10-month commitment and I agree to pay for the full session registered, unless I provide written notice of withdrawal in accordance with the terms and conditions set forth in the School Handbook. I agree to abide by the terms and conditions set forth therein.
* If you choose to withdraw, written formal notification must be provided to avoid incurring additional fees on your account, such as costumes that have not yet been ordered.
*Upon withdrawal, if a dancer’s costume(s) have been ordered, Morton Street may attempt to cancel the costume order. If cancellation is not possible, you will remain responsible for all costume costs incurred.
10 MONTH PROGRAM:
A non-refundable deposit is due at registration to reserve a space in a class(es). There are three (3) registration levels, with varying eligibility dates: VIP, Premium, and Standard Registration. The earlier you register, the more likely you are to secure the most appropriate class placement(s). Registration fees are separate from tuition.
All students/accounts must have a valid, current credit or debit card on file. You may opt to pay by ACH but a credit card is still required to be on file. You can register by paying the registration fee and costume deposit(s). The first tuition payment, the registration fee, and costume deposits must be made by the account holder in order to begin classes. MSDC will not handle the first payment by auto debit; that is incumbent on the account holder. All students who register before September 1st, must complete their registration by September 1st by providing payment in full or the first of eight the tuition payment installments and payments for tuition any applicable costume deposit(s). Failure to complete registration may result in the forfeiture of a reserved class placement(s).
Auto payments will begin on the 1st of each month from October 2024 (September 1st for 9-month pans) through April 1st, 2025. Any declined credit and debit cards or incomplete monthly payments will charged a $40 late fee on five (5) days after the due date. of the month. If an account is more than 5 days late, the dancer(s) will not be allowed to take class. Unfortunately, there are NO exceptions.
*Accounts with outstanding balances (including tuition, costumes, or other fees) after April 1st, 2025 will be charged a $50 late fee on April 5th, 2024 and the 1st of the month every month thereafter until satisfied. Unpaid balance are subject to collection, whether overdue or resulting from withdrawal.
Costumes and accessories will not be distributed to students with any outstanding balances on their accounts--no exceptions.
* Accounts with outstanding balances will also not be able to reserve ad space in our concert program.
____I understand that I am committing to a full 10 MONTH PROGRAM and will be responsible for all tuition and fees during that term.
____There is a $50 costume deposit per class due at the time of registration for each class. Costume balances are due by Saturday, February 15, 2025. Late costume and tuition payments will jeopardize receipt of the costumes.
____Attendance and timeliness in classes are essential so instructors can properly, developmentally and professionally train our students; attendance will be taken at the beginning of each class.
____* A student more than ten minutes late may be asked to observe class. Students must be present and prepared for class (in uniform with proper shoes, if requisite and with hair up) for class at least ten minutes before their scheduled class time. Students who are not prepared, as described, may be asked to sit out of class.
____ Attendance at all rehearsals, before any performance, is absolutely required. Any student who misses a final tech or dress rehearsal before a performance may be unable to participate in that performance, at the complete discretion of the instructor. Instructors reserve the right to re-stage choreography or remove a student from choreography because of excessive (3 or more) absences in either the Fall or Spring semesters.
____I acknowledge receipt of the 2024-2025 Season Calendar of Events and agree to attend each mandatory rehearsal. This calendar is available at all times through your MSDC Account (you must consent to this schedule when you submit your Online Registration).
2024- 2025 CORONAVIRUS/OTHER INFECTIOUS DISEASE PROTOCOLS FOR FAMILIES AND STUDENTS
During the COVID-19 Public Health Emergency (PHE) or any significant surge(s) post (PHE), the Coronavirus Protocols preempt any conflicting statement(s) in the Handbook.
PROTOCOLS:
-Properly fitting MASKS may continue to be required at this time. Two layer masks are required at a minimum. Three layers may be better. Face shields may also be required should the COVID numbers climb significantly. MSDC reserves the right to impose additional protocols, if necessary.
Class Sizes and Attendance
-Access to the dance studio is limited. Only students, faculty and staff will be allowed in the studio.
Students should arrive at least 10 minutes early. Students are asked to please enter the studio with limited items. Dance clothes should be worn under street clothes (Street clothes should always be worn outdoors over dance clothes). Street shoes may not be worn anywhere in the studio, including on carpeted spaces. Outside shoes may not be worn anywhere in the studio, including on carpeted spaces. Outside shoes must be removed at the door. Dancers will be advised where to place their personal items.
Classes are limited in size, so early registration is highly recommended.
2024- 2025 WAIVER AND RELEASE AGREEMENT
In consideration for receiving dance instruction provided by the Morton Street Dance Center, Inc. (hereafter sometimes referred to as MSDC or the dance school), my child and I agree to all of the following, which shall bind me and my child, and also my heirs, assigns, executors, and administrators. MSDC agrees to provide dance instruction as contracted, and the student/parent agree to the terms set herein.
Unless I notify the school, in writing, within 14 days of my/my student’s registration, I consent to the unrestricted use of any photographs or video taken of me or my child by or on behalf of Morton Street Dance Center, Inc.
I am not aware of any medical condition my child has which would preclude my child from participating in dance activities. I understand there are risks of injury associated with dancing and that such injuries include, but are not limited to bruises, dislocations, broken bones, and torn or damaged muscles or ligaments. I understand that such injuries may be caused in whole or in part by the student (my child), or by the actions or inactions of other students or instructors whether instruction is provided in person or via any online or virtual platform. I further understand that there are risks associated with the transmission of viruses or other diseases, including but not limited to COVID-19, in the dance studio or any other location where instruction or activities related to dance, dance instruction, or the dance school, that may occur.
I agree to instruct my child to immediately stop performing any activity if he or she feels any pain, dizziness, lightheadedness, or any other symptoms, and to immediately report those symptoms to the instructor. I also agree to instruct my child to carefully follow the instructor’s directions, to ask for additional directions if he or she does not understand any particular activity, and to immediately stop any activity which he or she is not competent or confident enough to perform. I also instruct my child to follow my directions and take every action to reduce the transmission of or the possibility of contracting a virus or disease, including COVID-19.
To the fullest extent permitted by law, I agree and represent that I and my child assume the risk(s) and responsibility for any and all injuries, illness or death sustained or resulting from dance instruction or related activities, any virus or diseases contracted by me, my child, friends, family members or others with whom I or my child come in contact with or the transmission of any virus or disease from me or my child to any other person while performing, engaged in, or as a result of any dance class or related activities. These assumed risks include any and all costs and damages, without limitation, that are a consequence of such injuries, virus or disease.
I and my child agree to defend, indemnify and hold harmless MSDC and their instructors, agents, employee, contractors, clients and students, from and against all claims, costs, damages, losses and expenses, including but not limited to attorney’s fees, arising out of or resulting from my child’s performance or other participation in any dance or dance-related activities, regardless of whether or not such claim, cost, damage, loss or expense was caused in part by a party indemnified hereunder.
I HAVE FULLY READ AND AGREE TO ALL OF THE ABOVE POLICIES AND TERMS AND I MAKE THIS AGREEMENT, THESE REPRESENTATIONS, AND CONSENT TO ALL OF THE TERMS, THE COVID-19 PROTOCOLS, AND WAIVERS AND RELEASES HEREIN IN ORDER MSDC TO ACCEPT MY CHILD AS A STUDENT FOR DANCE INSTRUCTION. I ALSO ACKNOWLEDGE RECEIPT OF THE 2024-2025 SEASON CALENDAR OF EVENTS.
