*I acknowledge that Momentum Arts Studios and or it's teachers/staff are not liable for any injuries and or loss/damages incurred to a guest or participant. In the event of the school being unable to contact me, I hereby consent to the medical treatment being sought for my child/myself if a medical emergency arises.
Where an ambulance is required I agree to pay all costs incurred.

*I hereby give permission for Momentum Arts Studios to take photographs, videos, film or audio recordings of myself/my child. I consent to these materials being used for promotional purposes by Momentum Arts Studios.
