I hereby consent to allow Boxcar Stage Company to use photographs, audio recordings, and/or videos of my child for promotional or educational purposes.

I understand that the photographs, audio recordings, and/or videos may be used in various forms of media, including but not limited to print materials, websites, social media, and advertisements. I also understand that my child's full name may be used in these publications.

I grant Boxcar Stage Company the right to crop, edit, and modify the photographs, audio recordings, and/or videos as they see fit for the intended use.

I acknowledge that I will not receive any compensation or royalties for the use of the photographs, audio recordings, and/or videos, and that I waive any right to inspect or approve the finished product or any use to which it may be applied.

I further release and discharge Boxcar Stage Company and its agents, representatives, and employees from any and all claims, demands, or causes of action arising out of or in connection with the use of the photographs, audio recordings, and/or videos.

I understand that this release is binding and will remain in effect until revoked by me in writing.

I hereby consent to allow my child, to participate in dance and theater classes at Boxcar Stage Company and release Boxcar Stage Company and its owners, instructors, employees, and agents from any liability, claims, damages, or causes of action arising out of or in connection with my child's participation in these activities.

I understand and acknowledge that there are risks associated with dance and theater classes, including but not limited to physical injuries, strains, sprains, and other bodily harm, and that Boxcar Stage Company has implemented safety protocols to minimize these risks.

I agree to assume full responsibility for any injuries or damages my child may incur while participating in dance and theater classes at Boxcar Stage Company, and to hold Boxcar Stage Company, its owners, instructors, employees, and agents harmless from any liability, claims, damages, or causes of action arising out of or in connection with my child's participation in these activities.

I acknowledge that I have read and understand this waiver and release of liability, and that I am signing it voluntarily and with full knowledge of its significance.
