I, on behalf of myself and my child(ren), hereby acknowledge and agree that on behalf of my child(ren), I accept and assume any and all risks associated with his/her/their attendance and participation in the Camp and all related activities. I understand that my child(ren) should not attend the Camp if he/she/they are not in a healthy condition. Furthermore, I do hereby verify that my child(ren), to the best of my knowledge, is free from contagious disease, is fully immunized, and is able to participate in the Camp. Therefore, I hereby confirm that my child(ren): (i) has no psychiatric, medical, and/or physical condition or history which would prevent him/her/them from participating in the Camp and/or endanger himself/herself/themselves or others at the Camp; (ii) has not been instructed by any physician to refrain from participating in the Camp; and (iii) is physically fit and able to participate in the Camp.

I acknowledge that, despite precautions, participation in dance activities is potentially dangerous and there is an inherent risk of injury involved. In my and/or my child's participation in Ballet Garden, LLC and Unity Dance & Movement activities, I hereby assume all the risks associated with the performing arts. I am aware that engaging in dance instruction is at my or my child's own risk.

I hereby declare any physical/mental problems, restrictions, or conditions and/or declare the participant to be in good physical and mental health. If I/my dancer has any particular injuries or concerns, I will disclose this concern to the instructor prior to class. I will ensure that I/we will exercise caution in all movements and that if dancing remotely/virtually the dance space is set up as a safe space for dance movement.

Session-based classes will be billed for the full session. You will be invoiced once your registration has been reviewed and any discounts or credits have been applied. Online registration requires a credit card on file and an email address. Payment may be made via Credit Card (Visa, Mastercard, Discover, and Amex), Debit Card, Paypal, ACH Transfer, or Check. Payment plans are available. Auto payment may be established for those who agree to e-payment processing. You must contact the studio directly to establish a payment plan. A non-refundable, one-time registration fee is due at the beginning of the first enrolled session of the school year (September - June). The registration fee is $100 for youth session classes. Tuition in all inclusive!

Please note the following Administrative fees:
Monthly tuition will be posted on the 1st of each month. Tuition is due on the 15th of the month.
A $25 Late Fee will be applied to any payments made 1 days past the due date. A $35 Bank Fee may be applied to declined payments, such as bounced checks, declined credit card payments, insufficient funds, etc. Drop-in class fees are due prior to the start of class or prior to receiving the recorded class, if dancing asynchronously with the live class. All sales are final for drop-in classes; no refunds will be given.

Ballet Garden, LLC and Unity Dance & Movement reserve the right to cancel any class with insufficient registration (less than 5 students) and a full refund will be given. If a student sustains an injury or an illness that does not permit them to participate over an extended period of time (1 month or longer), a credit will be issued. A doctor's note is required to issue a credit. With prior notice, missed classes can be made up by requesting a link to the class recording. No credits or refunds will be offered for missed classes. Dancers that register after the session has begun will receive a pro-rated class fee for the remaining session. Dancers will not be permitted to register after the fourth week of the session.

I understand the importance of myself and/or my child following the instructions and rules set by their instructor/s, and I agree to release Ballet Garden, LLC, Unity Dance & Movement, and its employees of any and all liability that may arise as a result of participation in activities with Ballet Garden, LLC and Unity Dance & Movement. I agree that I will not hold the studio, staff, or instructors liable for any injuries sustained during dance instruction, virtual or in-person. I/we will not hold Ballet Garden, LLC or Unity Dance & Movement liable for any personal injury or property damage caused in whole or in part by my/our participation in Ballet Garden, LLC and Unity Dance & Movement programs. I will ensure that I/we will exercise caution in all movements and that when dancing remotely/virtually, the dance space is set up as a safe space for dance movement.

I understand that when I participate as an online dance participant, I/my child are not permitted to screenshot, illegally share, or record virtual instruction without express permission from Ballet Garden, LLC and Unity Dance & Movement. To ensure privacy and security, virtual instruction will be recorded. I understand that my dancer's class recordings will be utilized at the discretion of Ballet Garden, LLC and Unity Dance & Movement.

I, the undersigned, give Unity Dance & Movement permission to send promotional emails to my email address.
