I have received or will obtain a copy of Dixon Dance Studio policies and will take the responsibility to carefully read and follow the rules and policies therein. Dixon Dance Studio reserves the right to refuse service to any student, parent, or legal guardian for any reason, including, but not limited to, not adhering to the rules set forth in these policies and procedures. Furthermore, I understand that Dixon Dance Studio does not give credit and/or refunds for class(es) missed due to holiday, vacation, illness, weather, etc.

I give permission for images of my child, captured at Dixon Dance Studio or at Dixon Dance Studio related events through video, photo and digital camera, to be used solely for the purposes of promotional material and publications, and waive any rights of compensation or ownership thereto.

I further understand that there are specific risks of physical or property damages, losses, or injury that may result from my or my child’s participation with Dixon Dance Studio. In recognition of this, I agree to release Dixon Dance Studio and hold them harmless of all liability, and hereby acknowledge that I am knowingly and voluntarily assuming full responsibility for all risks of physical injury arising out of active participation in a dance class or other dance related activities.
