I hereby grant Salsa y Control ("SyC") the irrevocable right and unrestricted permission to use and publish photography and/or video images of the Student(s), or in which the Student(s) may be included, for any purpose authorized by SyC, including but not limited to: repertoire website, social media, editorial publications, video recording, and advertising. This includes the right to modify and retouch the image at the discretion of SyC. I understand there will be no compensation to me or the Student(s) for this use. Furthermore, I understand that I will not be given the opportunity to inspect nor approve the finished products or printed/published matter that may be used in connection therewith. In granting permission to SyC and its representatives, I am fully and without limitation releasing it from any liability that may arise form the use of the images. Salsa y Control (SyC) works to provide a safe environment for all dancers; however, with dance, as with other physical activity, there may be certain associated risks.
I hereby forever release and waive my right, for myself and/or the Student(s), to bring suit against SyC and its owners, agents, and employees in connection with participating in and using SyC's premises and/or services, including dance instruction, classes, and other activities. I understand this waiver means I give up my right to bring any claims on behalf of myself and/or the Student(s), including for personal injuries, illness, medical treatment, death, disease, property losses, or any other loss, including but not limited to claims of negligence, and I give up any claim I may have to seek damages, whether known or unknown, foreseen or unforeseen.
By signing below, I acknowledge:
1) I have carefully read and fully understand all provisions of this release; and
2) I freely and knowingly assume the risk and waive my rights concerning liability as described above for myself and/or the Student(s).
