My child/children has my permission to attend activities, events, competitions, etc. relating to Encore Performers Inc. and release it's owner, staff, instructors, choreographers, landlords, or lessors from any and all liability for injury to my person, my children and / or any charges or damage to property, whether based on allegations of negligence or not; in any way incurred by reason of my participation or that of my children /charges in any class, program, activity, event, competition, etc. offered at or by Encore Performers Inc.
I hereby authorize Raynor van der Merwe or any adult she deems appropriate to take necessary actions in the event my above-mentioned child, needs medical attention while attending activities, etc. relating to Encore Performers Inc.
I/We the undersigned hereby agree to give a one month notice if we intend to stop dance classes, and I/We agree to abide by the Studio Information & Policies of 2024-2025.
Annual Registration fee per student: $45 (Annual Reg. Fee is valid through June 2024 and non-refundable) Costume fee: $90 per class. Payments made by credit card will assess additional fees. All families will be charged a recital media fee for a digital copy of the recital performance; $20 for students in 1-2 dances, $40 for students in 3+ dances.
Encore Performers reserves the right to use any images of your child, including but not limited to photos and videos, for promotional, marketing and advertising materials (this includes posting on Encore Performer's website and other social media pages).
