I give Dance Time, LLC permission to use my child's photo and video for marketing purposes. I understand that no royalty, fee or other compensation shall become payable to me by reason of such use.

I recognize and understand the risks of physical injury inherent to dancing and I fully assume those risks for myself and my child(ren). I hereby release Dance Time, LLC and its dance teachers from all liability for injuries sustained while participating in any dance classes or events.
