I HEREBY ASSUME ALL OF THE RISKS OF PARTICIPATING IN THIS ACTIVITY OR EVENT, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective equipment or property owned, maintained, or controlled by them, or because of their possible liability without fault.

I certify that I am or my dancer(s) physically fit or have sufficiently prepared or trained for participation in the activity or event, and have not been advised to not participate by a qualified medical professional. I certify that there are no health-related reasons or problems which preclude my participation in this activity or event.

In consideration of my application and permitting me to participate in this event, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows:

I HEREBY WAIVE, RELEASE, AND DISCHARGE VIRTUOUS DANCE CENTER and all divisions thereof of any and all liability and responsibility for injuries, sickness, pandemics, accidents, natural disasters and/or acts of God incurred during participation in and/or instruction of camps, intensives, private instruction, choreography or any activity I may participate.

I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from this event, THE FOLLOWING ENTITIES OR PERSONS: VIRTUOUS DANCE CENTER and/or their directors, officers, managers, employees, volunteers, representatives, and agents, the activity or event holders, activity or event sponsors, activity or event volunteers;

I INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in this waiver, release and registration form from any and all liabilities or claims made as a result of participation in this activity or event, whether caused by the negligence of release or otherwise.

The accident waiver and release of liability shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.

I Understand that As a parent, guardian or student of Virtuous Dance Center your child may be involved in activities and events, from which photos (analog and digital), video, and printed material may be included in publications, including newsletters and promotional reports and videos, on VDC website (www.virtuousdancecenter.com), and for educational purposes. In addition, books and other creative works designed by staff and students may be displayed in schools, at school- sponsored events. VDC supports the positive efforts, activities, and honors of our students, which involves working with outside news media, including local newspapers, radio, and television stations, to promote these accomplishments.

PAYMENT POLICIES FOR DANCE CLASSES TUITION
First month tuition is due upon registration. All other months are due on the first of each calendar month. Payments received after the 5th of each month are consider late and will incur in a $35 late. Any student with an overdue balance of more than 30 days will be dropped from enrollment. Discounts can not be combined. New students joining in the middle of the month will have their class pro-rated for that month only. If a student drops a class after the first class of the month, tuition in non-refundable. Accounts must be paid in full to participate in the recital. We offer automatic bill pay online payment options and you can set up that on our online portal.

I have read or will obtain a copy of Virtuous Dance Center policies by request only and will take the responsibility to carefully read and follow the rules and policies therein. I understand that Virtuous Dance Center and the Dance Box does not give credit and/or refunds for class(es) missed due to holiday, vacation, illness, weather, etc. I understand that by the end of every season all balances pending must paid in full and give permission to Virtuous Dance Center to charge credit card on file for any balances due or not paid by the end of the season. Balances not paid will be forwarded to a Collection Agency to request payments due.

PRIVATE CLASSES: Any cancellation must be made 48 hours before the scheduled lesson. Lessons cancelled or rescheduled with less than 48 hours’ notice will be charged a $35 fee. If you cancel a lesson after the start time of the lesson or miss your lesson without any notice, you will be charged the full price of the lesson. Private lessons are non-refundable.
GROUP CLASSES: There is a withdraw fee of $75 dollars per student with a 30 day grace period. If a student decides to withdraw from a class, a 30-day prior written notice must be provided via email. If 30-day prior written notice is not provided, the account will continue to be due and payable. Tuition will be adjusted for the next month’s payment. No refunds will be given for withdrawn classes. While we do not offer refunds, you are welcome to use any studio credits towards a different class within one year. Lesson packages and gift certificates expire one year from the date of purchase. They are non-refundable.

I CERTIFY THAT I HAVE READ THIS DOCUMENT, AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT OF MY OWN FREE WILL.

PARENT / GUARDIAN WAIVER FOR MINORS (Only if student is under 18 years old)

The undersigned parent and natural guardian does hereby represent that he/she is, in fact, acting in such capacity, has consented to his/her child or ward’s participation in the activity or event, and has agreed individually and on behalf of the child or ward, to the terms of the accident waiver and release of liability set forth above. The undersigned parent or guardian further agrees to save and hold harmless and indemnify each and all of the parties referred to above from all liability, loss, cost, claim, or damage whatsoever which may be imposed upon said parties because of any defect in or lack of such capacity to so act and release said parties on behalf of the minor and the parents or legal guardian.

Electronic Signature Agreement. By selecting the "I Accept" checkbox, I am signing this agreement electronically. I agree this electronic signature is the legal equivalent of my manual signature on this agreement.

I have had sufficient opportunity to read this entire document. I have read and understood it, and I agree to be bound by its terms.
